Now Dean's starting to get a little annoyed because, as much as he loves the kid, Sam is not the best person to share a bed with, he moves around and makes noises and clings, so Dean gets him his own hotel room, just so he can get some sleep for once. Sam is alive, has lost the wall, and is somewhat functional. Dean and he have settled down. Dean's adapting and starting to build friendships, but he notices that Sam is still aloof and on-guard. When he begins paying closer attention, he realizes that Sam is constantly testing everyone they interact with for demon possession. (Sam remembers Lucifer's reveal in 5x22 that he's always had a demon or two in his life, and doesn't want to risk it happening again.)

Gen, please, any season
I've been reading Sabriel by Garth Nix, so...hm....let's see some NECROMANCY! Or something like it.
Anyway, the basic idea comes from the description in Sabriel of how the Dead beings called Mordicants are created: combining clay and human blood and a ton of evil magic, then adding a soul to the mix.
So what if somebody wants to give Sam a "gift," the gift of the soul of somebody he loved and lost? And what if that somebody is Jessica? Maybe it's a random witch with some talent for necromancy who's sorry to hear about Sam's dead girlfriend and misguidedly tries to help, but isn't really powerful enough to actually resurrect her. Or maybe it's a demon (or Lucifer) who wants to either gain Sam's favor or gain some sort of leverage over him. Completely up to you.
Bottom line, though, there's Sam and Dean, stuck with Jessica's soul, which has been ripped out of heaven and is literally stuck in a little jar made out of bloody clay and dark magic. Needless to say, Sam isn't handling it well, falling helplessly in love all over again, only to be reminded by Dean- over and over again- how screwed up this all is, and how she shouldn't be here at all. They find a way to send her back to Heaven so she won't be trapped forever, but the thing is, Jessica isn't so sure she wants to go back, having figured out that her heaven is nothing more than a loop of false memories, and is delighted to see Sam again, even when she figures out all that he'd been hiding from her, and sees how her death and life on the road with Dean had changed him. And Sam knows it, but he doesn't know what to do.
Take liberties if you want. Just wanna see Jess back, however briefly. :D

Sam gets hurt badly enough during a hunt to need a blood transfusion. But this is before anyone told him about having demon blood in him and how that can complicate things. The net result is that the transfusion goes very wrong very quickly. He gets really sick, his body starts rejecting the new blood, or whatever you want. (There are a bunch of ways that transfusions can technically go wrong, which you can wiki if you want to find symptoms to use, but it could be wierd and unexplained too.)
Gen please, can be at any point before Sam finds out about having Azazel's blood in him, so pre-series or during series, whenever. Just make sure a certain SOMEBODY is really worried about him. :)

So what if those demons Sam grew up around thought John wasn't training Sam up right and constantly tried to kidnap him for their own purposes. Of course, this means weird stuff keeps happening around Sam, and some hunters get involved...

John gets steadily more paranoid, freaked out, and over-protective. Dean is just trying to calm everyone down, and Sam is mostly oblivious, but completely frightened by John's new temperament.

Sam's not used to riding a horse and after getting back from 1861, his ass and thighs are sore as hell. Dean notices Sam's walking funny and makes fun of it for it until Sam snaps at him and Dean realizes he's really in pain. Then Dean insists on giving Sam a soothing massage. Sexy and dirty times ensue.

Kinda related to crazybeagle's post earlier:

Sam gets admitted to the hospital for a traumatic head injury, and the doctors start up with their tests, blood work, CT skull scans, ECG monitoring, EKG monitoring, temperature monitoring, the whole nine yards, and find things wrong with him. Not like compressed skull fracture wrong with him: really freakishly wrong with him. Incompatible with human life wrong with him.

So Sam is woozy, disoriented, pumped full of muscle relaxants, nauseous, shaved half bald, and alone, and the doctors are all circling the room, watching him like he's some newly discovered deep sea creature, asking question after question after question . . .
It's all news to Sam, and he just can't stop thinking about Azazel, Ruby, and Lucifer, and what kind of unholy abomination he's become now that they've all had their hands on him. And where the heck is Dean?

wee!Sam discovers a hunting knife. Dean yells at him (in fear and worry) and startles Sam who drops the knife, slicing himself open rather badly.

John has to stitch Sam up while Dean has to try and keep him still, whilst trying to comfort him at the same time.

I put this in a meta post, but now maybe I don't so much want to write it as want someone else to write it so I can cry a lot:

If Dean were to die on Sam now I don’t think Sam would go crazy or vengeful or try to open hell gates or drink demon blood or anything. I think he’d even try to go on, knowing that that’s what Dean would want and really, really not wanting to fuck up Dean’s last wishes again. But I think he’d just fade, get killed by some random monster, not out of any suicidal drive, but just because he was having trouble paying attention to details like food or sleep or ravening creatures with lots of big, nasty teeth. (Yes, it is possible that I now want to write this fic, what with being a sick individual. The one where Sam parks in the motel parking lot and pauses a moment to get up his energy to go into the office and gradually realizes that it’s morning and he’s been sitting there all night staring straight in front of him and he never actually got out of the car and got a room.)

Set S1. Sam and Dean are having one of their typical bickering, alpha-male, arms waving arguments. It's over nothing, really, just some silly thing that happened over breakfast. Trouble is, Sam's starting to freak out. It's nothing that Dean said, and yet it's everything. All of a sudden, Sam breaks away, runs into the bathroom, starts screaming his head off. He's sobbing uncontrollably, shaking like mad, can't stop clawing at his skin like he's covered in bugs, can't breathe despite his constant hyperventilation, and he's sure that he's going to die. Sam's never had a panic attack before, and it's utterly terrifying. His mind is slipping away before his fingertips, his vision's going hazy, his skin's about to fall off, he's spitting up blood, and he can't breathe.

Dean finally gets him to open the door, half an hour later, and can't understand why their fight made him so upset. Sam has to explain to Dean, through the remnants of his panic, what happened. Dean tries to calm him down.

(Long prompt is long, but this is drawn from personal experience. Suffice it to say, yesterday was a very sucky day.)

Divergent post-6.11. When Sam wakes up from being resouled, the last thing he remembers is getting stabbed at Cold Oak. Dean and company are tasked with deciding what to tell him regarding the apocalypse, Lucifer, et al., and the fact that angels exist! Sam has to deal with all these changes, getting used to the fact that Yellow-Eyes is dead, coming to terms with his personal responsibility for ending the freaking world, and just generally the fact that he's missing more than 4 years of his life. Some of the stuff that's happened is just too ridiculous. Dean himself wouldn't believe it if he hadn't experienced it. Season six gen, after Sam's soul is returned. Sam is injured on a hunt and has a concussion. He keeps mentioning a voicemail and the amulet. Things are resolved between the brothers.

In response to [livejournal.com profile] klutzy_girl's prompt above, pre-series, seventeen-year-old Sam has been harboring secret feelings for Dean since he was in the ninth grade. When it doesn't wear off, he plans to kill himself. Dean finds out before he can attempt it and stops him. But he never gets over the anger he feels at Sam for scaring him so badly, and never admits that's part of what keeps him from calling Sam at Stanford.

Years later, when he and Sam are on the road again trying to find their dad, he finally confronts Sam and Sam admits the truth. Could be mutual, or not, you decide.

John is possessed by YED at the end of season 1, but the boys don't figure it out right away. John acts normal when Dean's around, but is downright mean to Sam when they're alone. Having had such a huge fight with his Dad and having always felt like a disappointment to him, Sam really believes it's John who is saying/doing these things to him, but chooses not to tell Dean because he's embarrassed and because he doesn't want to screw up Dean's family reunion. As time goes on, possessed John's behavior gets worse and worse until it's both physical and emotional abuse. Eventually Dean finds out what's going on and knows his Dad is possessed. He just can't believe that Sam didn't realize. Afterwards, John is horrified both by his behavior and by the fact that Sam believed it was him and let it continue for so long.

Gen or Wincest - Sam cracks his pelvis. What can I say - I have a thing for Sam's hips and apparently I like to break the things I like. Bonus points if Dean makes fun of him for "breaking a hip like an old man" but is actually very concerned and caring. :)

Wincest - Sam and Dean are having sex and it goes all wrong. Sam falls off the bed and then manages to slam his head against the head board when he and Dean try again. Dean feels guilty and goes overboard in trying to help him recover.

Gen or Wincest AU of "The End"- In "The End" when Dean tells Sam that they should stay away from one another, Sam takes it to heart. He cuts off all ties with Bobby and any other mutual acquaintances in order to avoid inadvertently having contact with Dean. Not because he doesn't WANT to see Dean (he does) but he knows that Dean doesn't want to see HIM, and he's pretty sure he's a curse to anyone who cares about him. So it's just better that he disappear. Dean doesn't call him back after his time travel experience, and time just keeps moving on. Eventually Dean (who is missing his brother) starts fishing for tidbits about Sam from Bobby. But Bobby doesn't know WHERE Sam is - hasn't heard from him in ages. Turns out, no one has, and Dean freaks the hell out and tracks Sam down. What he finds is a sad, lonely man who has cut himself off from all unnecessary human contact. Dean realizes how badly it hurt his brother when he cut him out of his life. Cue the rebuilding of a brotherhood and of Sam's sense of self.

You know that point you get to when you're REALLY sick with the flu, where you're SO sick of puking and feeling awful that you basically revert to a 5 year old? You just want to feel better and for the puking to end, and you don't really care how irrational it is to be begging someone else to make it stop - you just want it OVER. You're pretty sure you cannot take a single second more of feeling so crappy, and you might be embarrassed about the sniffling and moaning and general patheticness later, but right now you JUST DON'T CARE.

Well, that's the point that Sammy's at, and it's Dean's job to make things better. I want feverish, sweaty, pukey, MISERABLE Sam begging Dean to make it better in between episodes of dry-heaving, and caring, paternal Dean putting cool cloths on the back of his neck and rubbing his shoulders while he pukes.

(Remember how we got glimpses of natural disasters and big, destructive things happening during the brief time Lucifer was possessing Sam, but never really focused on that stuff?)

Sam's always spent a lot of time online, researching this and that, even when there's no immediate hunt. Dean doesn't think there's anything significant about it now. Though Sam's getting obsessed. Dean wakes up at three, four in the morning to the glow of the laptop screen and the tap of the keyboard. You'd think resouled!Sam wasn't sleeping any more than robo!Sam did. He's getting circles under his eyes, zoning out when Dean's talking to him. He'll eat a bite or two of his dinner and let the rest of it get cold while he goes back to the computer.

Then Dean stumbles on Sam's files. Every article he could find about those disasters. Pictures of smashed houses. Casualty lists. People who would be alive now if Sam had gotten Lucifer into the cage a little sooner.
