Still want to see this, so reposting it...
Sam gets hurt badly enough during a hunt to need a blood transfusion. But this is before anyone told him about having demon blood in him and how that can complicate things. The net result is that the transfusion goes very wrong very quickly. He gets really sick, his body starts rejecting the new blood, or whatever you want. (There are a bunch of ways that transfusions can technically go wrong, which you can wiki if you want to find symptoms to use, but it could be wierd and unexplained too.)
Gen please, can be at any point before Sam finds out about having Azazel's blood in him, so pre-series or during series, whenever. Just make sure a certain SOMEBODY is really worried about him. :)

So what if those demons Sam grew up around thought John wasn't training Sam up right and constantly tried to kidnap him for their own purposes. Of course, this means weird stuff keeps happening around Sam, and some hunters get involved...

John gets steadily more paranoid, freaked out, and over-protective. Dean is just trying to calm everyone down, and Sam is mostly oblivious, but completely frightened by John's new temperament.

Sam's not used to riding a horse and after getting back from 1861, his ass and thighs are sore as hell. Dean notices Sam's walking funny and makes fun of it for it until Sam snaps at him and Dean realizes he's really in pain. Then Dean insists on giving Sam a soothing massage. Sexy and dirty times ensue.

Dean sighed, thinking his words over. Sam held his breath.

And then Dean said the last thing Sam would even have thought he'd say.

"I'm sorry."

Sam let out the breath he was holding with a gasp of incredulity. "You...wait, what?"

Dean glanced up at him, and there was that look again that Sam still didn't understand. "Yeah, you. The look on your face when I tossed you your jacket. Or when I said I needed to talk to you. You just..." Dean turned away. "Not a look I ever wanted to see on your face again," he said quietly, and for half a second Sam was certain Dean was looking elsewhere, remembering something. And it wasn't something good.

When he turned back to Sam, Sam realized what the look on Dean's face was: regret. "And I managed to put it there, somehow," Dean continued, and when he smiled this time, it was small and tinged with sadness. "So you gotta tell me what I did-"

"No," Sam immediately corrected, his stomach twisting again. God, none of this was Dean's fault at all. "I mean, no, Dean, it wasn't your fault."

"Yeah," Dean said, but his voice was firmer. "And you're still trying to take the fault for something that's not yours. God, it's like you're-"

Dean froze. Sam froze. He wasn't exactly sure why he did, but he knew whatever came out of Dean's mouth next wasn't going to be good.

"You are," Dean said softly, as if just realizing something. He huffed an incredulous laugh before shaking his head. "Oh god, you are. Of course you are."

The urge to get up and flee had never been stronger, but Sam hadn't so much as given energy to his muscles before Dean was focused on him again. "You listen to me," he said quietly, but it wasn't a resigned tone, or a defeated one. Not even the upward swing of elated that Dean had taken for months when he wasn't one of the others. No, this was all big brother who knew what was right and what was wrong and how the world worked. This was the big brother he'd kept believing in.

"You've been gone for...awhile," Dean said vaguely. "And I've had some time to pull my head out of my ass, all right? You were gone, Sam. Straight down that hole into the Pit and..." He swallowed convulsively and blinked hard for a minute. "And I didn't think I was getting you back," he added. "But I did. And it's not gonna be like it was before, all right? This is it. No angels between us, no demons, nothing. This is us, the Winchesters, just doing the family business. And I'm not doing it without my little brother. I fought hard to get you back, and I'm gonna fight just as hard to keep you."

It was Sam's turn to swallow hard and turn away. The hand that fell on his shoulder this time definitely wasn't reluctantly put there as an obligation. It felt like the rest of the times Dean had caught him or patted his shoulder for the past few days: it felt like a reassurance that Sam was still there, and that Dean was still there and not going anywhere.

Just as Dean was about to pull his hand away, Sam's hand shot up and caught his brother's tightly. Dean gripped back just as hard. "It's over, Sammy," Dean said softly.

And for the first time since he'd opened his eyes and found his brother hovering above him, not the darkness as he fell, Sam truly believed it.

He glanced over at his brother, his big brother, and Sam gave a genuine smile. "I know," he said, and Dean grinned back at him.

It was over. And they were picking up right where they'd left off.

Kinda related to crazybeagle's post earlier:

Sam gets admitted to the hospital for a traumatic head injury, and the doctors start up with their tests, blood work, CT skull scans, ECG monitoring, EKG monitoring, temperature monitoring, the whole nine yards, and find things wrong with him. Not like compressed skull fracture wrong with him: really freakishly wrong with him. Incompatible with human life wrong with him.

So Sam is woozy, disoriented, pumped full of muscle relaxants, nauseous, shaved half bald, and alone, and the doctors are all circling the room, watching him like he's some newly discovered deep sea creature, asking question after question after question . . .
It's all news to Sam, and he just can't stop thinking about Azazel, Ruby, and Lucifer, and what kind of unholy abomination he's become now that they've all had their hands on him. And where the heck is Dean?

wee!Sam discovers a hunting knife. Dean yells at him (in fear and worry) and startles Sam who drops the knife, slicing himself open rather badly.

John has to stitch Sam up while Dean has to try and keep him still, whilst trying to comfort him at the same time.

I put this in a meta post, but now maybe I don't so much want to write it as want someone else to write it so I can cry a lot:

If Dean were to die on Sam now I don’t think Sam would go crazy or vengeful or try to open hell gates or drink demon blood or anything. I think he’d even try to go on, knowing that that’s what Dean would want and really, really not wanting to fuck up Dean’s last wishes again. But I think he’d just fade, get killed by some random monster, not out of any suicidal drive, but just because he was having trouble paying attention to details like food or sleep or ravening creatures with lots of big, nasty teeth. (Yes, it is possible that I now want to write this fic, what with being a sick individual. The one where Sam parks in the motel parking lot and pauses a moment to get up his energy to go into the office and gradually realizes that it’s morning and he’s been sitting there all night staring straight in front of him and he never actually got out of the car and got a room.)

So they pushed and they shoved at the door and searched the walls, but nothing else was in there besides them and a few frozen chunks of meat hanging from the ceiling.

As soon as they sat down, though, Sam started acting different. "You okay?" Dean asked, watching as Sam shifted on the ground, trying to get comfortable.

"Yeah," Sam said, then shifted again. "Just...cold."

"And gonna get colder," Dean said, and Sam shivered. Dean frowned but stuck to his side of the freezer. It wasn't that big, and even on the opposite side of the wall Dean could easily tap Sam's boot with his own. "You want my jacket?"

Sam didn't respond right away, eyes focused on his hands. "Hey, Sam," Dean said, concern growing. He tapped his foot against Sam's one more time.

Sam blinked and jerked his head up. "Yeah?"

"You want my jacket?" Dean repeated, still frowning. Cold or not, hypothermia didn't set in that fast, and that was the only thing that could slow someone's reaction time like that.

Sam made a face and shook his head. "You're gonna need it," he said, then glanced towards the meat hanging from the ceiling. They were long, perfect slabs for punching, and Dean imagined himself having a Rocky moment, complete with his own theme song.

"Hey, Sam? Wanna hum the theme to 'Rocky' for me?"

Sam didn't respond again. Dean turned his gaze from the meat to Sam and found his brother staring at the meat with growing trepidation on his face. His hands kept falling to rest on the floor, then jerked up and away, only to fall back down and repeat the whole process again.

"Sam," Dean called. Sam didn't move. "Sam," Dean called again, more fervently this time. Sam still didn't move or give any indication that he'd heard his brother.

Concern was quickly growing into tiny tendrils of panic. "Sammy," Dean said, and kicked hard at his brother's boots. Sam's legs fell over onto the floor, and Sam hissed in surprise and fear before he curled them up against him.

All the while his eyes never left the slabs of meat.

Dean pushed himself off the floor (and god was it cold against his hands) to sit beside his brother. "Sammy," Dean called again, and he knew there was panic in his voice now, but his brother was attempting to go comatose in a freezer and hey, not cool, Sam. "Sammy, talk to me."

"So cold," Sam whispered, and Dean was so surprised to hear him respond that he almost missed with Sam said next. "I expected fire, you know? But it was just cold."

Oh god. Oh god no.

"Sammy, look at me," Dean said urgently, his fingers tapping on Sam's frozen cheeks. "Sammy, look at me, dammit!" Because he didn't have to look at the meat to know exactly what Sam was seeing. He'd been to Hell. He didn't think much was different in the Pit.

Except it had probably been Sam on the hooks, trussed up like meat, and if Dean finished that thought he was going to throw up.

Sam finally allowed his head to turn away from the meat. "Look at me," Dean insisted, trying to lock onto Sam's gaze. Sam dutifully looked at him for half a minute, and then his gaze went glassy, and Dean knew he could talk until he was blue in the face. He wasn't going to keep Sam's attention there with him until they were out. The icy floor, the cold air, all of it was going to keep reminding Sam of where he wasn't, of where he used to be.

Dean had to get them out.

Cas said that Sam's soul had been skinned basically. The physical consequence is that Sam's very raw emotionally.

Tall guys constantly get roped into changing light bulbs when shorter humans would need to get a ladder -- like the bulbs in the twelve-foot kitchen ceiling at Jess's parents' house.

Sam really should have known better than to stand on a swivel chair to do it.
