Ok, seeing as others are posting prompts that didn't get picked up last time, I'll put this out there again. Especially as I can see it fitting in well with the current season 6 timeline and character relationships.

Sam is poisoned/cursed/contracts a supernatural illness of some sort. The illness is not fatal. Without treatment, Sam will eventually recover, but he'll be out of commission for weeks (or maybe even months) with fever, chills, extreme weakness, and/or whatever fun H/C symptoms you want to throw in there.

There is only one known cure, but it involves demon blood.

Sam has a choice. Take the cure and face another round of withdrawal and guilt, or suffer through a miserable illness and slow recovery and feeling useless/helpless.

Sam's Wall comes down for whatever reason. This leads to seizures, followed by Sam in a coma. Cas is able to determine that the problem is Sam's soul is locked deep within his own mind, and is so badly damaged by the reintegration that he can't find his way out. Cas thinks that he will be locked in forever unless someone can guide him out. Dean asks Castiel to send him into Sam's mind (bonus points if this only works because Sam and Dean are soulmates!). Dean has to find his way through the Hell-scape of Sam's inner life to to find him and bring him home.

So, between all the rage and grief early on, and all the moving around and obsession with the hunt later on, what are the chances that John didn't really go out of his way to keep the boys up to date on all their childhood immunizations? I'd like to see some consequences of this for Sammy. Could be wee!chesters, teen!chesters, or even later, as lots of these diseases are worse for adults. Heck, could even be post 6x18, god knows what could be picked up during that one. ;-)

Sam and Dean plus Cas or Bobby or whoever else. Gen. Sam's wall comes down and he has to deal with not only the memories of hell and soulless Sam but also with being able to see, hear, smell etc. everything for what it really is. (For example he is able to pick out a demon or angel without them revealing themselves and see hidden sigils and signs and hear angels speaking etc). His new abilities overwhelm him and cause sensory overload and Dean and whoever else have to deal with a freaked out, panicking Sammy while trying to find a way for him to be able to cope with it.

In the Ancient Greek Underworld, bad people (or people who offended the gods) were tortured forever in Tartarus (which wasn't a demon factory, you have to give it that much); good people (or people the gods liked) lived it up in the Elysian Fields; everyone else wandered around aimlessly in the Fields of Asphodel. Said wandering aimlessly was facilitated by giving them water from the River Lethe, the River of Forgetfulness, which made them forget they had anything to do but wander around aimlessly.

So... Lethe makes you forget, or at least forget to care about, the past. And it's meant for use on shades, not living people, so it might not even have an effect on a living body.

Sam's body was only in Hell for a few days, Earth-time. Giving his soul amnesia might (according to theory) not affect his memory of anything his body experienced. It might or might not (no one knows) muck up some of his reactions a la Souless Sam, but his soul would be intact so he'd be able to feel and able to learn. It might or might not have who knows what other side effects -- but it would erase the suffering and crazymaking material behind the Wall.

Of course, it's from a river in the Greek underworld, and the more things that leak past the Wall to be experienced whilst embodied, the less effective it will be...

Sam had a painful skin condition, Psoriasis, on his hands and knees and neck (Filler can choose - im not too fussed =D). Most days, with perscribed cream, it's alright but when they've been hunting a while and Sam hasn't had a chance to get some medication it gets bad quickly, until it's hard and cracked and he lies awake at night unable to sleep from the itching and the pain and lets not talk about actually using his sore hands and knees D=.

The wall in sams mind doesnt crack, it's explodes into a million shards of pain. But that's not the worse of it - the worse of it is knowing that in hell, he wasn't tortured by the devil - but instead given a gift by him to relive his life as if Jessica was never pinned to the ceiling and burnt alive, that Dean never went to hell. Perhaps the most painful thing, is that he wants to go back.

Dean had to convince Sam that it was torture all the same, and only things here and now are real.

This is similar to an earlier prompt I saw here (I hope that doesn’t step on any toes)...

Sam, Dean, John, Sam -- gen

Sam (season 5 or 6) time travels to the 90’s and starts hunting with his family under an alias. Sam is about fourteen/fifteen then, and very obviously hates hunting. teen!Sam wants Dean to back him up in arguments with John, complains about his family, and just has normal teen angst.

adult!Sam has so much rage/hate/guilt against himself that he targets it on teen!Sam, giving him the cold shoulder and calling him whiny/brat/etc, or being just generally verbally mean to him. This doesn’t go over well with Dean (or John) and Sam feels pretty alienated and hurt that his family is turning on him (not that they realize he's Sam!). Both Sams are unhappy.

Sam has an allergic reaction (I’m thinking more hayfever, no food allergies please) on a hunt. Maybe he’s allergic to the MotW, maybe its seasonal allergies, and the boys are somewhere in the middle of nowhere with no access to meds, so he has to tough it out. Cue Dean doing anything to make him feel better.

I've been discussing Sam and Dean meeting up with the Brewsters with Brose D'Shield (http://brosedshield.livejournal.com/26723.html?thread=106083) and would love to actually see a fic here. I'm not really sure how to spoiler text here (or if I really need to for a movie as old as this one) so I'm sorry if this doesn't work.

If you haven't seen this, it probably sounds a bit odd but is actually hilarious and is one of my favorite Cary Grant movies.

This could be with both Sam and Dean but I was also thinking it would be perfect for when Sam is alone in early season 5. I'd prefer gen and not a death fic.

Gen. Sam is blind and gets teased and harassed. At one point he's apart from Dean and has to ask someone for directions and they intentionally misguide him so that he ends up in the wrong place, totally lost (maybe be ends up in an alley or behind a building or somewhere else vacant). The jerks have followed him (or led him there?) and circle him and start taunting him, maybe they take something from him and keep passing it around asking him who has it now, if he can guess they might let him go, or something. He tries to grab for it where he thinks it is but then they get full on aggressive and start beating him. After they leave he manages to get up and try to find his way back but ends up somewhere precarious and falls and/or gets stuck or something. Dean finally finds him- maybe one of them calls the other or Dean asks people on the streets if they've seen anything (because Sam's not answering either because he can't or has no reception etc). Cue caring and P.O.ed big brother.
Can be pre-series if you like but still adult Sam (i.e. no younger than late teens).

Strangulation

Header says it all: Sam gets strangled. And strangled. And strangled.

How is up to you: pissed off spirit, another psychic using telekinesis, demons, vamps, you name it--just so long as Sammy gets the daylights choked out of him. Rough the boy up but good, anything short of brain damage. And soothe the poor guy afterward, of course.

Sam/Dean or gen.

-Bonus points for: setting it in either S2 or S6; avoiding hanging; screwing with his head in some way (like, it's a possessed Dean who strangles him or something).

Tornado hurt/comfort

I admit this is inspired by the disasters yesterday across the U.S., which are very serious and horrible and I don't mean to trivialize at all. I hope no one's offended by me using it as a prompt.

Preseries, Sam about 11 or 12. The Winchesters are staying somewhere in the open Midwest, prime tornado country, in some terrible clapboard house that wouldn't stand up in heavy winds, let alone a real tornado. John's only supposed to be gone a couple nights, and of course that's when the tornado sirens go off. At first Dean scoffs, but Sam's turned on the radio and after listening to the urgent warnings and also all their neighbors clearing out - maybe a police officer going down their street (which are all flimsy low-income houses for rent), warning them they need to evacuate, telling them to head to a nearby sturdy...school gymnasium or warehouse or something. Then Dean packs their duffels and pulls Sam along with the rest of the crowd, and they stay out the night in the sturdy place, listening to the wind and things smashing outside. Sam isn't really scared now (though he might have been earlier, before Dean was taking it seriously), but Dean has his arm around him tight the whole time, especially with so many strangers packed so close around. They probably lose power at some point, too.

Then in the morning, when they finally deem it safe, the boys go back to find their house flattened. Ripped apart, nothing left but the foundation and maybe the toilet, traces of plumbing. And there's chaos, people frantic, maybe some rescue workers urgently asking where their father is and Dean has to make something up, but they don't know where to go to wait for John to return. They end up back at the relief station, which is probably in a gymnasium, full of cots and emergency supplies, and that's where John finally finds them.

I'd like for John to be less of a bastard in this fic - as soon as he heard about the tornadoes, he turned around, but he had trouble getting back because of the trees and power lines down over the roads, the National Guard in the way. There should be a fairly heartwarming reunion scene, John shouting for them across the crowded gymnasium and rather fierce clutching-hugs when he reaches them. And then Dean asking anxiously if the Impala is okay.

Uh, making this about Sam again: he doesn't freak out, he's fairly strong and keeps it together, but nevertheless it's pretty unsettling when the place you'd been living in (even if only for a couple days) is completely demolished and Dean's only 15/16 and you don't know how your dad's going to find you, and meanwhile everything else around them is completely torn up, people missing and bleeding and dead. So he is kinda scared, and I think during/after the tornado, he gets very very quiet. Hardly talks at all and keeps very close to Dean, who isn't about to let go of him either, afraid to lose him. I'm a sucker for clinging. :)

Sam & Ouija Board

Gen-Sam uses a Ouija board to try to contact John for some reason (such as for help with the deal Dean made or whatever). Unbeknown to our beloved hero, it is not John he is conversing with but a malevolent spirit/demon posing as John, and the entity begins to wear him down/control him. Cue emotional torment, insomnia, hallucinations, loss of appetite, increasingly bizarre/unhinged behaviour, possible eventual possession etc. Go nuts :-) Oh, protective, worried Dean of course!
