My personal view of the situation is that even though Dean does have major issues with insecurity in this area (I agree with Sam that his big-time butchness is certainly a manifestation) he would bury them just like he buries everything else for the sake of the Prime Directive, looking after Sammy even when Sammy goes and tells him something like this.
