This is beautiful, beautiful, and so realistic. It's been hard for me to imagine what happened after Jess died, but this could be it. And I love how you work in Sam and Dean's history, relationship, everything. So many beautiful bits - I love every time Dean touches him. I love how he's nearly catatonic right afterward, how he only responds to direct commands, how angry Dean gets when he nearly has to strip Sam (and angry too at Jess for coming between them in a way), how worried he is about him.
Possibly my favorite description:
He reaches out, then, hands brushing over his brother's neck, his jaw, up toward his face. "Sammy," and it's almost sing-song, pitched too soft, but Sam turns toward Dean's hands, towards his voice, and Dean says, "it's okay, Sam." His fingers trace lines and tension, over and over, and something builds in Dean's chest, sucks the air out. Laughter, Dean thinks, but it's too heavy, too choked. "Sam."
And this: Dean moves his right hand, finds the back of Sam's neck. It's always been his spot, that brief touch, to offer comfort or understanding when he couldn't always use words. "I'm sorry," and he feels Sam shake under his hands.
And all the things he doesn't say, between the lines, under the other words. It's perfect.
