It shouldn't have been a big deal. Dean and Sam had squatted in boarded up houses plenty of times. This shouldn't have been any different. This time, though, they needed a warded room. Before it could be warded, though, the walls and floors had to be cleaned. Dean left the cleaning and warding to Mr. Neat Freak OCD symbol dictionary and went out to interview witnesses. It isn't until weeks later that the symptoms start, and it isn't until days after that, when Sam can't breathe, that they finally understand the consequences. Hanta virus.

For the time being, with the accumulation of violations, he doesn't want to invite anyone across his body boundaries, period, but even if that changes, that's no longer a possibility for him with Dean. How do he and Dean work out proximity and forgiveness in the shadow of a no longer tenable past involvement? If you wish, let Sam go on to a tentative sexual relationship with someone else (Cas or not!dead!damn!it!Kevin or Jody would be my favorite picks), and look at how Sam and Dean's relationship weathers that development. Basically, I want to see Sam's sexuality impacted by his experience of body/self/consent violations, combined with post-wincest Sam n Dean.

Curious as to why they are different he draws on on the floor but when he tries to cross through it, he finds himself trapped. Trying to not panic as he waits for Dean to get back to the Batcave, Sam realizes that he feels exhausted and the longer he's in the trap the weaker he gets, these traps were designed to weaken demons not just hold them.

Dean thinks having a fluffy little were-kitten for a brother is hilarious, and amuses himself by trying to startle Sam on the odd occasion just to watch it happen - but one day he legitimately upsets Sam enough for him to transform, and then can't find him afterwards.

S9 AU: Sam cast out Lucifer, ffs, even with the memory-wiping (maybe especially with the memory-wiping) he had to realize something was off. He figures out how he survived the trials and forces Gadreel out, knowing that he may not survive. Whether he survives is up to you. Of course, Dean is there to try and talk Sam out of it.

Sam Winchester becomes disillusioned and mentally borked from everything that's happened to his head. As a result, he believes he's in a line of work, or thinks he's in a certain situation (i.e. he's a firefighter, he's fighting imaginary fires, he's dressing up to go rescue something out of a tree; you can use firefighter or whatever the hell pops into your head). Dean isn't about to take his crazy-as-balls brother out into the world where someone'll lock him in a nuthouse, so they live in a house in the woods, where it's nice and peaceful and he can remind Sam x1000 of who he is and bring him back to reality, keep him normal Sam for 5 goddamn seconds.

Cue the daily life of making sure his little brother doesn't break his neck or get lost or whatever.

Sam/OMC (Sam/Dean or Sam/Cas also works), set in season 9 or sometime thereafter (post-7.17 when Cas heals Sam also works)

Post-Gadreel!Sam hasn't been interested sex for months now but eventually he meets an OMC (or Dean very slowly wins back his trust or Castiel begins to woo Sam) who tickles his fancy (can be at a bar one night, someone he met on a case, another hunter, whatever) enough to want to try again. When they make it to a bed, though, Sam finds he's shaking and anxious but tries to push himself (maybe after everything he's been through, he feels like his consent/willingness isn't that important or he doesn't want to let himself "fail" at this step towards healing), OMC doesn't let him though and tells him they don't have to do anything Sam doesn't want and instead they spend the night doing nothing more than kissing and heavy petting with OMC being patient and slowly building up to more while periodically asking if Sam is okay, if it's all right to touch him a certain way, etc and Sam surprises himself by how much he ends up enjoying it. How far they end up going is up to author, so long as there is no dick-in-bum action.

Aww, that was so cute! Poor Sammy. It's not actually that hard to imagine him not knowing the normal things that happen with kids because they were always on the move.

I love how you added that Sam could feel that he was different somehow. Nice.

Season 9. Ever since he was able, Sam's liked eating healthy. It's one of the few things in his life he could control: what went into his body. After the Gadreel violation though, Sam takes it to another level and starts eating less and less. He's losing weight and lethargic to the point where he gets hurt during a hunt. Cue Dean/Jody/Cas realizing what's going on and stepping in. They try to get Sam to talk about his feelings, encourage him to eat, and give him loving support.
Bonus points for guilty!Dean and D/J/C struggling with how to help Sam and snapping at him which sets Sam back. Gen, wincest, sassy are all welcome.

season 8-9, gen or wincest. Sam gets hit with a truth spell and speaks his mind whenever Dean takes a cheap shot at him. Maybe he even starts crying after a particularly brutal hit. After seeing Sam's guilt, self-loathing, and suicidal ideation, Dean is chastised. Once the spell wears off, Sam is humiliated.

After 9x19 I've got a hankering for sweet little jailbait Sam being used as bait during a vampire hunt that goes wrong. Blood drinking, physical assault, sexual assault; however far author wants to take it is fine with me so long as it ends with Dean comforting a hurt!Sam. Gen or wincest.

A demon captures Sam and knows it is his birthday. It tortures him, ties him up, drops him in a box and wraps it up all pretty for Dean to find later. Feel free to use as many details of the Sam-in-a-box genre as you want, including Sam panicking in the box, injuring himself worse through struggling, puking on himself and the ever necessary Dean ribbing feeling back into his limbs after he arrives to pull him out.

Charlie is back from Oz for a quick visit. She and Sam get trapped somewhere dark and cold. Sam is badly injured or drugged and is really out of it. Charlie has to keep him from panicking while they wait for Dean.

Jody gets a panicked call from Dean. Sam has been marked by a monster and it's stalking him. Sam has been injured and can't defend himself. Dean needs Jody to take care of Sam while he goes off to gank the monster.
