Castiel says he can "shift" Sam's Hell memories, and that his solution will get Sam "back on his feet." "Back on his feet" to me implies a kindasortamaybe fix, one that isn't perfect. Maybe Sam can function better that he could before, and can generally get around, but he's still not quite alright. It's up to you how severe the 'not-alright' is. Dean has to come to terms with the truth that his brother is never going to be perfectly mentally well again, but he's awesome about it. Gen, please.

Bonus points if Sam, very calmly and very sanely, explains that really, Dean, everything's fine, he was meant to be this way.

Sam is in the hospital. A staff member / visitor / another patient notices that he has dissociative tendencies. Whenever the stress gets too much, or certain things come up, he becomes slow, disoriented, sleepy, and uncoordinated. He can't remember basic things and maybe slurs a little and is generally just... a little out of it. When it gets bad, he may even become unresponsive, drifting somewhere in his head. But he is still trying to hold it together, because... you choose: Dean is around? He vaguely knows that he needs to do something important?

Sam/Dean or gen, trigger warning for non-con/sexual abuse, PTSD - Castiel was able to shift the worst of Sam's memories and psychic damage to himself, but could not take all of his hell-related trauma. Part of what he left behind was the memories of . Physically there were worse things he experienced in the cage, and the whole overwhelming volume of traumatic memories had sort of bled together into one indistinct mess of pain and blood and fear. But with the most gruesome memories out of the way, Sam's memories of are suddenly more clear and distinct. Before, those memories were just one drop in an ocean of pain, and in a strange way that made them less horrific because trauma was all Sam knew. But now that he's more clear-headed, suddenly those memories seem more overwhelming and horrible compared to his now mostly-sane existence. He finds himself really struggling with them, despite having Hallicifer out of his head. He doesn't want to tell Dean, because his brother is just so glad that he's "fixed" and Sam doesn't want to burst his bubble, but soon he just can't hide the nightmares and flashbacks anymore and he breaks down. He tells Dean what's going on, and Dean helps him process what was done to him and move on.

Sam's immune system is shot because of his insomnia and he catches a bad cold. Dean has to leave him in Rufus' cabin to go get supplies. Bobby watches over Sam and tries to take care of him while they wait for Dean to get back.

Sam+Dean gen, TW for child abuse/sexual abuse and repressed memories.

Cas took Sam's memories of hell quickly, in crisis-mode, and with not a lot of finesse and preparation. As a result, he disturbed some of Sam's pre-hell memories as well (like doing surgery with spoon instead of a scalpel because that's al; they had time for). Sam had spent his whole life repressing terrible memories about a traumatic event in his childhood when he was but Cas inadvertently unearthed a bit of those memories and started a domino effect that leads to Sam fully remembering what happened, bit by terrible bit. Dean doesn't understand what's happening and is worried that it's the hell memories seeping back, but eventually he gets clued in (Sam talks in his sleep? Flashback? Sam tells him? etc) and does his best to make things better (supporting Sam, helping him deal with the emotional fallout, maybe even going after the hunter who hurt him?). Would love it if Sam isn't in that "angry/lashing out/don't fucking touch me" state of mind, but rather he really wants Dean's support and affection and seeks it out (if you can do that without making him too woobie, that is!).

There are more demons outside than Dean first thought, giving the demon with Sam more time to torture him. By the time Dean and Cas get to Sam he's stopped breathing from the convulsions caused by the ECT and they aren't sure how long he's been with out oxygen.

The ECT didn't seem to effect Sam but a few weeks later Dean notices that Sam still flinches at things that aren't there, zones out or has trouble with his balance. Dean keeps a close eye on Sam but doesn't say anything till one day these episodes lead to a full blown tonic-clonic seizure and a life time of seizure medication for Sam.

7.17 crossover AU: The only hospital in the area that has a free bed in the psych ward is the VA, and they take Sam only because it's an emergency. It turns out, though, that there's a guy next door to Sam who knew John back in 'Nam--an Army captain, true, but sometimes the Marines needed backup and the Army choppers got there first. Sam's thrilled, Hallucifer or no, and Dean's kind of relieved when the captain offers to keep tabs on Sam for him while Dean tracks down a healer.
But it seems they don't call the man "Howlin' Mad" Murdock for nothing.

Gen, please, and I prefer original TV canon for The A-Team. Bonus points for BA and Face dropping in to give Sam some real comfort, whatever shenanigans he and Murdock get up to. Double bonus points if BA can get Hallucifer to shut up for any length of time at all.

Sam got Electric Shock Therapy. It worked.

Problem is, now he doesn't remember anything. No idea who he is, who he is supposed to be. Dean starts trying to jog his memory of their life before Sam went to Hell, but that's a dangerous game to play when Lucifer's just waiting in his subconscious to be let in again...

What if Marin wasn't in the hospital at all or was a figment of Sams mind & Sam stopped eating altogether due the sandwich incident? Maybe he grows weaker because he stops eating or the hospital ends up having to forcefully feed him... it's up to the author... Either way it doesn't help Sams hallucinations or recovery...

I prompted something very similar at hoodie_time, but it's a favorite trope, so I want it for Sam, too.

Hallucifer gone, huge relief, right? Sam's not seeing stuff and he can sleep and his organs aren't failing.

Except he's been so focused on carrying on, functioning in spite of Hallucifer, so much of his drive has gone into that, that when Hallucifer's not there to fend off any more it's like the thing that was keeping Sam going is gone. Functioning ought to be easy now, but suddenly he's just out, he can't, he's done. He crashes, hard. And just when it seems like he's got no resources left, all the other accumulated damage he wasn't dealing with because Hallucifer was more urgent is becoming harder and harder to ignore.

Hallucifer sang a lot of Deanish music. Sam doesn't do too well with Zeppelin and co. in the aftermath. On the other hand, there's not much left that actually makes Dean happy, last thing Sam wants to do is make him change the station. And he hates that Hallucifer took those bits of Dean and made them his.

It would be nice if there were stress association induced migraines in there, but I'm not picky.

So, this time Sam's really out, right? He's not in the Cage. He knows he's not in the Cage. He's hunting with his brother. There's something still going on, maybe, but it's Cas who's experiencing it now, right? Sam's finally clear, no misgivings, no doubt glimpsed out of the corners of his eyes.

This may be Lucifer's best mindgame yet. After all, you can't torture someone who's got nothing left to lose.

When the paramedics arrive at the scene of Sam's run-in with the car, he's totally confused and out of it and he more than freaks out a little, fighting back as they strap him to the backboard because in his mind, they're strapping him to a rack and planning on torturing him. It doesn't help matters either that Lucifer is along for the ride and trying to convince Sam that they aren't taking him to a hospital, but back to the cage.

I'd really love to see a fic from the point of view of a sympathetic nurse in the hospital, trying to make this tall sad young man named Sam hurt less, and watching him slowly deteriorate.

Castiel couldn't take the crazy onto himself since he's not human. Be he COULD transfer SOME of it to another human. Dean, of course, insists he give some of Sam's crazy to him....Now both boys are plagued by the crazy and have a shared Halucifer they can both see....but because the crazy was split in two, they're not too far gone to function. Sam is horrified Dean did this to himself, and is horrified that, since Dean shares his hallucinations, which often stem from memories, Dean is seeing or hearing about all the torturous and humiliating things that happened to him in the cage.

During the time Hallucifer was keeping Sam from sleeping. He's more and more starting to see things. They're in a diner and Dean is ordering something at the counter. A cop bumps into Sam, and recognizes him, possibly from a recent documentary on the Winchesters. Before Dean can do something, the cop radios in for backup, Sam tries to stop him, but Lucifer is distracting him too much.

Anyway, utterly by accident they end up taking everyone in the diner hostage,while surrounded by cops and Sam is slowly breaking apart. The big problem being, they can't just let the hostages go, because the cops outside, terrified of the horrible Winchester brothers are too trigger happy and would start shooting at the slightest movement towards the doors.

And all the while Sam's not sure if what he's seeing is real, or not.

The trouble with being plagued with compound hallucinations for such a long period of time is that it's difficult to tell what's real and what isn't. Case in point? There is no Marin. She's just a facet of Sam himself and the ghost he's putting to rest,
