Castiel says he can "shift" Sam's Hell memories, and that his solution will get Sam "back on his feet." "Back on his feet" to me implies a kindasortamaybe fix, one that isn't perfect. Maybe Sam can function better that he could before, and can generally get around, but he's still not quite alright. It's up to you how severe the 'not-alright' is. Dean has to come to terms with the truth that his brother is never going to be perfectly mentally well again, but he's awesome about it. Gen, please.

Sam is in the hospital. A staff member / visitor / another patient notices that he has dissociative tendencies. Whenever the stress gets too much, or certain things come up, he becomes slow, disoriented, sleepy, and uncoordinated. He can't remember basic things and maybe slurs a little and is generally just... a little out of it. When it gets bad, he may even become unresponsive, drifting somewhere in his head. But he is still trying to hold it together, because... you choose: Dean is around? He vaguely knows that he needs to do something important?

Okay, really, the hallucinations are gone and the memories blunted like a dull knife, but you can still do a lot of damage with a dull blade. If you try. If you really press in with it. Sam takes the dull edge of memory and he digs because while it's awesome that he can sleep, awesome that he doesn't see and hear Lucifer everywhere, he's also found and lost Cas in seconds and that's not tolerable. Cas is in there, in his head, remnants left behind and it hurts to dig for him, scares him, scares Dean, but Sam keeps digging at it like he's trying to get at a deeply embedded splinter.
Gen or Sam/Cas. Either or! (No Dean sucks for leaving Cas in the psych ward, if you please.)

The transfer was a few seconds, bits of memory, wisps of shredded soul, a bit of grace, a touch of humanity, tears and blood, all of these things went back and forth between Sam and Cas in seconds, but the moment they stood before one another - Cas, Sam, and Lucifer - in that space, that universe that could only exist for as long as Sam and Cas were joined, went on for days, maybe weeks. A blink of an eye outside in our world, but much longer in theirs. You get the idea. Gen or Sam/Cas. As surreal and star crossed/angst ridden as you please.

Hooray! Sam's fixed and all better! Except when he lies back to sleep or just as he wakes, he hears the sound of a child crying. It's soft, the kind of crying made by a kid that doesn't want to be caught out at being a baby. Sam doesn't say anything because he's Better and No Longer Full of The Crazy and More Normal, but it's kind of making him quietly nuts. Turns out the sound is himself, the old memories of his childhood that Lucifer had shoved to the side to make room for all his fire, brimstone, and grandstanding, are slowly coming back - including those of a small, confused kid who only had his big brother to really rely on.

ECT results in two types of memory loss. The first involves rapid forgetting of new information. For example, shortly after the treatment, patients have difficulty remembering conversations or things they have recently read. This type of memory loss is short lived and has not been shown to last for more than a few weeks after the completion of ECT.

Sam can't remmeber what he just read or what he had for breakfast. He can't entertain himself or do research. He's frustrated and feeling like he's useless. So, every night Dean reads him the same chapter from whatever book strikes your fancy and tells him that it'll pass. It's a relief to finally move on to the next chapter when Sam improves, but some part of Dean is irrationally sad, too, because this quiet little hiatus while Sam screws his head back on right has been kind of nice.

Sam has spent so long trying to hold his head together and function. thats why he externalized hell in the from of Hallucifer. Now that lucifer is gone Sam can clearly remember hell. This is ok, he can handle the memories now, without going insane. But it's only just hit him how long he was separated from Dean and how much he missed him.

Cue weepy, sleepy Sam and awsome Dean.

After being broken out of the mental institution, Sam is touch-dependent on Dean to keep him functioning and sane. The memories of hell are gone, but the damage is still very real.

Sam was separated from his brother for 100 years in the cage. Sure he'd never see him again. He was tortured in every way imaginable and worse.

Somehow, while trying to hold it together in the months since the wall fell he's managed not to *feel* it. In fact Hallucifer embodied all the emotion he was trying so hard not to feel.

Now that he's gone it all hits him at once. A breakdown and confession like Dean had at the end of "Heaven and Hell." Maybe Dean actually lets his defences and talks to Sam about his own time in Hell.

Sam/Dean or Gen. God wants Cas whole, that's been made apparent time and again. The 'shift' is temporary, puts Sam back on his feet long enough to finish things with the Leviathans, but once it's over Sam has to take his burden back.

Resistance to THAT much sedation is not normal, and instead of dismissing it the doctors decide to study Sam, which only leads to them finding more strange things about him and more and more painful and intrusive tests. Lucifer is amused.

The whole mental hospital thing and even Sam getting better? None of it's real. Yup, Sam is still in the Cage and this is all part of Lucifer's elegant torture. And he's just getting started.

Cas said he could never truly fix Sam. He told Dean he could 'at least get him back on his feet'. And in an interview that came out before the episode, Jared said what got Sam out of the hospital was just basically slapping a brace on and limping back into the fight. AKA, Sam's not fixed.

Cas could only shift the majority of the damage from Sam to himself, but it wears off. Cas starts getting better and Sam starts getting worse again. Cas can't keep fixing Sam even if Sam would let him (which he won't), Lucifer's vessel, demon blood, etc... for whatever reason, angelic powers are ever less effective on Sam. So they're pretty much right back where they were. Cas and Dean try and take care of Sam.

Cas and Sam are connected now. After all, Cas was rummaging around inside a dangerously hell-shattered mind. Sam knows whatever Cas is going through now is because of him. It doesn't take him long to figure out that he can help the angel. Sam knows when hallucinations build up and overwhelm Castiel and learns that if Sam hurts himself, he can ease Cas's pain for awhile. Cas doesn't realize what Sam's doing at first, but when he gets it and is lucid enough, he alerts Dean. Needless to say, neither of them are pleased. They all try to pull together and try to help each other.

Sam can't stand the fact that Castiel is now suffering in his place. Already emotionally frayed and with hell and Lucifer quieter, but still not gone from his mind, the guilt is tearing at him horribly. He puts up a brave front for Dean, who he knows is hurting over Cas even if he won't admit it, but things quickly spiral out of control again. Sam can't sleep or eat much and Dean is forced to watch his brother slip through his fingers in an all new way. Dean desperately tries to help Sam. (Maybe by fixing Cas?)

Sam hasn't told Dean about his extracurricular research into getting Adam out of the Cage. The guilt was already eating at Sam and he didn't want to put that on Dean too. Now on lock down, an exhausted Sam is tormented by visions of his lost little brother, left behind. Sam protected Adam then, but now that he's gone, Sam knows Michael and Lucifer have no one else to take their rage out on.

Bobby is helpless, only able to watch them deteriorate. Every name crossed off of Dean's list, every hallucination and freak out Sam has only breaks the hunter's heart more.

Bonus if Bobby is utterly desperate to help Sam, frustrated at not being able to even touch him or offer comfort. Sam freaks again, yelling at Lucifer. Doctors swarm him and Bobby watches with tears in his eyes. As they pin Sam down, drug him, trap him, Bobby yells at the clearly empty space that had horrified Sam for Lucifer to leave him alone.

While in the institution, Sam's really out of it and starts hallucinating back to another asylum long ago....and somehow Dr. Ellicott is back. Hmmm....and maybe this time makes him shoot Dean for real (at least in his mind). When Dean shows up--alive--Sam is beside himself with relief, guilt, confusion and fear.

So, Sam's walking around and trusting what he sees because he let Castiel carry his crazy as part of some hopeless quest for personal redemption, and left him alone, cringing at the walls, in the care of a demon they know for a fact has some sick fascination with him. Sam and Dean are carrying on, hunting, with Castiel suffering in their wake. Sam's supposed to deal better like this.

Sam supposes, for all the use it's getting, he might as well pop his soul back out like a trick shoulder and at least get some peace.

Whether Sam does anything with this thought or just angsts about it is anyone's guess.

Sam can finally sleep, just not very well. He goes back to the tweaker for more "help". Dean finds out eventually. At first he thinks slurry Sam is just recouperating.

Besides helping Dean find help for Sam, Bobby watches over Sam every moment he's not with Dean, even though it breaks his heart to watch Sam slip further and further away. Desperate, exhausted, and drugged up Sam calls out for Dean again and again. During another episode, the doctors hold Sam down and try to drug him, unsuccessfully, once more. Sam screams and thrashes and cries and calls out for Bobby.

Cas tries to shift the damage and he fails. But he learns something -- that Sam's not really hallucinating afterall. Somehow, someway, Lucifer can communicate with his vessel while still in the cage and more than anything, he wants Sam to return to him. Cas can save Sam in the immediate by forcing him to sleep when necessary, but can do nothing to protect him from Lucifer when he's awake so Sam's mental decline continues. Dean is desperate to find a way to severe the connection between Sam and Lucifer before he drives Sam completely mad and/or succeeds in killing him. And also, Dean would really like if Cas would just stop telling him there is no way to break the bond between an Angel and its Vessel

So Hallucifer wasn't entirely a hallucination: turns out that psychic link still works from the Cage. Luci cobbled together stuff from Sam's subconscious to pry away at what's left of his mind, getting stronger as Sam got weaker. His game plan is to weaken Sam's mind enough that he can take over Sam's body. Dean and Cas get back to the hospital too late - the sleep deprivation and the ECT were the final straw, and Lucifer is in control. Some part of Sam is still in there, though, watching in helpless horror as Lucifer starts to carry out his plan...

What if Sam did wake up, but too late when he fell asleep behind the wheel in 7x16? Cue crash, hurt and maybe even big brother still on the phone to hear it?

Cas healed what he could, and Sam is physically healthy. All he needs is some decent sleep, lots of good food, and a regular training schedule, and he'll be back in fighting hunting shape in no time. Only Sam pushes himself too far, too fast and winds up flat on his back exhausted &/or injured. The good news is, he doesn't have to deal with Lucifer's malicious wit. The bad news is, he has to deal with Dean's over-zealous TLC and constant refrain of "I told you so."
