Maybe Sam can function better that he could before, and can generally get around, but he's still not quite alright. It's up to you how severe the 'not-alright' is. Dean has to come to terms with the truth that his brother is never going to be perfectly mentally well again, but he's awesome about it. Gen, please.

Whenever the stress gets too much, or certain things come up, he becomes slow, disoriented, sleepy, and uncoordinated. He can't remember basic things and maybe slurs a little and is generally just... a little out of it. When it gets bad, he may even become unresponsive, drifting somewhere in his head. But he is still trying to hold it together, because... you choose: Dean is around? He vaguely knows that he needs to do something important?

After so many weeks with Lucifer's constant chatter, Sam finds the slience unsettling. He tries listening to music, playing video games, but nothing helps except Dean's voice. Sam comes up with excuses to get Dean to read to him: case notes, stuff off the internet, articles from Busty Asian Beauties, anything at all.

While Sam was alone in the locked ward, the possessed orderly raped him - but with Sam's hallucinations in overdrive and Lucifer's memory constantly molesting him anyway, Sam doesn't realise it really happened.
Turns out the demon gave him an STD: Something that's treatable, but potentially fatal if ignored for too long. Sam never went to get checked for anything like that since he didn't think it was real.

Would also like Sam and Dean (especially Dean) freaking out when they figure out what happened to Sam.

Remember how Hallucifer tried to convince Sam back in 7.1-2 that he was still in the cage, and everything else was a hallucination? Ever since they left the hospital, Sam is fighing an ever-growing dread that it's true. Cas with amnesia, married to some woman named Daphne? Bobby choosing to be a ghost? Leviathan real estate agents curing cancer? Cas "shifting" his crazy? Meg showing up out of nowhere to "help" them? Dean choosing to abandon Cas to nurse Meg? Doesn't that all sound crazy in itself?

Sam becomes ever more convinced that nothing that's happening is real, Dean's not real, and Sam never escaped the Cage. Dean has to find a way to convince him otherwise. Maybe Sam engages in dangerous behavior or starts to shut down as a result of being convinced he's completely insane/still in Hell?

Sam has always had pretty un-brotherly feelings for Dean, but he took that secret with him to the grave. Lucifer, of course, pulls all of Sam's juicy secrets out of his mind. He's particularly delighted by that one, and one of his favourite ways to torment Sam is to sexually abuse him wearing Dean's face. Sam is pretty traumatized by this, but has kept it well-buried. When Dean is trying to take care of him after they get out of the hospital, maybe undressing him to put him to bed, he accidentally triggers a flashback, and Sam has a major meltdown. Up to you if Dean has secretly felt the same way about Sam or not. Could also work with previous consensual Wincest.

Sam knows better than anybody that grand, sweeping gestures of self-sacrifice never work out the way they were intended.

See, when Castiel did the Transfer? He didn't really know what he was doing (or perhaps he did, and he didn't care). What he took from Sam was not a memory, or a sliver of the devil, but an idea--a single, edifying manifestation of the worst possible thing that's ever happened to you (or the worst that you have done). Castiel's lived for millennia, has seen so many horrible things. He has fought many wars, and has much to feel guilty about and so much more to regret. Of course what he took from Sam was going to incapacitate him.

What Castiel didn't know was that in doing so, he created a permanent connection to Sam's mind. Sam, whose memories of the Cage are still intact, watches, tries to share the burden. When he sleeps, when he can see Castiel better in a kind of telepathic dreamscape, he stands with him against Lucifer. Even if it means risking his health. He can deal with the nosebleeds and migraines and the endless torture playing constantly against his eyelids the next day as long as he knows that he was able to fight back against that which tormented him for (centuries) months, and, even more importantly, that he was able to help his friend.

Castiel's 'shift' was successful, he has Lucifer in his head rather than in Sam's, but there's a problem. Removing Lucifer from Sam's mind has left a...hole and this hole causes Sam to suffer from terrible headaches. Cue bigbrother!Dean taking care of Sam and the both of them learning how to deal with this new problem.

Post 7x17-- Casitel said he couldn't fix what wasn't there. And he meant it. Sam is still having small issues, and hand-pressing still isn't working. Slowly, hallucinations begin to flow back, and this time it isn't just Lucifer. Comforting Dean for extra cookies!

The doctor said he'd seen nothing like it when the drugs wouldn't knock out Sam. The doctors find out about other aspects of his strange physiology.
Sam has demon blood in him. Sam has been locked in Hell with Lucifer for 180-some years. Sam was Lucifer's one and only vessel and Luci wore him to the prom. Some things just never change.

Probably way too late to be noticed, but here we go:


Towards the end of the ep, Dean and co. end up outside Sam's hospital, where they are surprised to find it surrounded by demons, but throughout the ep, the demons had been following "Emmanuel" to find out how he's doing the healing. Why do they already have the hospital surrounded unless they're there for a different reason?

It's Sam, of course, but I'll leave it writer's choice whether they're drawn by Sam's suffering, Sam's connection with Lucifer, Sam's powers kicking up, or another unexplored reason.

Gen, please.

Did you notice the scene where Lucifer's reading Sam a bedtime story? Between Cas' first attempt to heal Sam and the transfer of crazy? That happened in the cage too. At unpredictable times, cruelty would turn to kindness and Lucifer would comfort Sam like Dean used to when they were kids. At first Sam resisted, but when you've no hope, you take what you can get.

This is probably a bit more porny for H/C but after leaving hospital Sam is still having nightmares, even if he's not seeing things when he's awake, and that, excessive caffine and kidney issues he wets the bed. Some Sam/Dean slashy comfort from Dean (bed sharing - Dean trying to convince Sam he's okay with it and not disgusted, etc).

When Sam gets hit by the car and is taken to the hospital, the doctors discover not only about his mental problems, but also that he's pregnant [because it's normal for men, or for some men to get pregnant] and the baby miraculously survived the accident and the sleep/mental issues, so far...

When Dean finds out, he is desperate to find a way to save Sam and their unborn baby!

Gen, kinda sorta AU-ish 7x17:
Cas didn't get there in time to save Sam from the electroshock "therapy." The demon takes him, all broken now, for more "experiments." He hooks him up for a blood transfusion, only the blood that he's giving Sam is his own. Demon blood. Straight into Sam's veins.
Some bonus questions: Will he survive through it? Will anybody else survive through it? Not only how will Sam react, but how will Dean react to it?

Okay, this is a slightly tweaked version of a prompt I posted to a Dean meme, because I WANT THIS FIC SO BAD and honestly? I love both Winchesters enough to inflict fictional suffering on them equally. Please forgive. I have just had bad luck with prompts and really have my heart set on this one. /o\

So, basically, a lot of Sam being confused and vulnerable, maybe being herded around the ward by a well-intentioned nurse, obediently doing what he's told but not really registering much. He tries, but it's all too much for him, and his brain keeps shutting down without much warning.

The person who sees this may or may not recognize what's going on, but they feel for him as they watch him struggle to hold on to full consciousness, and they try their best to ground him, get him talking and give him comfort.

Gen please, could be 7.17 or any other episode this season.

Between the time Sam was hit by the car and when he was rescued, Sam was in and out of consciousness several times. However, the doctor said no matter how many drugs they gave him, they couldn't knock him out (or keep him knocked out). I need to know what was done to (or by) Sam while he was doped up, out of it, and/or stoned off his gourd.

Sam's only been out of the hospital a few days when he finds a hunt nearby that he claims can't wait, so when he falls asleep during the stakeout, Dean isn't surprised, or even upset. What does upset him, though, is that the thing has shown up and is making so much noise that it might wake Sam back up. Dean tries to kill the monster quietly, so that Sam can have his rest.

For whatever reason i like the scene from The Born-Again Identity with Sam and Marin where She's giving him a candy bar and Luci's talking on the loudspeaker(?) and sam keeps flinching at the loud sound.

I'd love a fic where even after Cas does the transfer Sam still has that twitch. Sam's frustrated that he has no control over this thing that happens to him randomly, Dean's all worried.

bonus points if Sam is embarrassed when it happen in front of other people.
