Maybe Sam can function better that he could before, and can generally get around, but he's still not quite alright. It's up to you how severe the 'not-alright' is. Dean has to come to terms with the truth that his brother is never going to be perfectly mentally well again, but he's awesome about it. Gen, please.

So one morning Dean gets back from grabbing breakfast to find Sam standing naked in the bathroom carving into his body where he knows he's supposed to have scars from Lucifer. Bonus points if Sam, very calmly and very sanely, explains that really, Dean, everything's fine, he was meant to be this way.

Sam is in the hospital. A staff member / visitor / another patient notices that he has dissociative tendencies. Whenever the stress gets too much, or certain things come up, he becomes slow, disoriented, sleepy, and uncoordinated. He can't remember basic things and maybe slurs a little and is generally just... a little out of it. When it gets bad, he may even become unresponsive, drifting somewhere in his head. But he is still trying to hold it together, because... you choose: Dean is around? He vaguely knows that he needs to do something important?

Oh god this is my very first time leaving a prompt. Well...

“Sam/Dean(doesn´t need to be Slash, can be Gen too). But the injuries to his fingers and hand make shaving and shampooing nearly impossible and he's almost in tears over how frustrating it is. this is more a post 7x17 bot ok...

Remember the Xmas special, and that female deity taking off that nail? Sure must have hurt; I would like to see a fic were Sam literally bites his nails, or ripped them off (maybe to keep Lucifer away or something or he was too far gone to notice). Now he’s on the road with Dean and he has to wait for his nails to grow back.
And it’s really, really painful, since in his line of work, they keep cracking.

Gen please: Electroshock therapy isn't recommended for people that are prone to seizures. But a full-blown seizure is usually followed by a period of unconsciousness.

Gen please: It actually takes months to die of insomnia, as seen in the real-life cases of Fatal Familial Insomnia. However, long before it's deadly, the insomnia causes hallucinations and even waking dreams. Dean comes to realize he never wanted to know what his brother dreams.

I am half awake, but her are a few ideas I had...
Sam purposefully ran into the street to get hit by a car (suicide attempt, just everything to stop)

2. When Sam get's 'healed' he starts to face the results (is high)/withdrawal for whatever drug he took

Sam so sleep deprived he is acting drunk; slurred speech, horrible balance, spacing out. Dean starts noticing it and Sam tries to hide that he hasn't been sleeping.

When Sam finally shaves, he's so out of it and sleep deprived that he accidentally shaves off his sideburns in addition to his beard. Afterwards he is very sad.

I apologize for this prompt. I've been trying to cut caffeine and it has left me out of my mind.

Maybe his immune system is wonky from sleep deprivation, maybe it's a holdover from his time with Lucifer, maybe it's something else, but Sam starts spiking ridiculously high fevers for no good reason. Every time it happens Lucifer returns with a vengeance and Sam has to relive his worst memories from the cage. Bonus points if Dean is all like 'Hell to the No! This is not suppose to happen' but is still all wonderful and caring and manages to talk Sam down from his freak-out. Gen, please.

"Lucifer has convince Sam, telling him that he is pregnant and Dean has left him because Dean cant stand something unnatural like that. At this point, Sam is so tired and full of drugs that belive what Lucifer says. He just want Dean to come back and accept him, and his "baby" " Can be Gen or slash.(Crappy English)

Post 7x17 - Sam is having trouble with food still (after seeing things in his food because of Lucifer) and is finding eating a serious problem. Maybe he's refusing to eat, or maybe he's suspicious about everything he eats. Dean helps him out - making sure food is good/exciting, or making sure he knows how it's being prepared or something, up to you. Gen or slash is fine (hey, making food 'exciting' in sexy ways might be good).

After Castiel "cures" him, Sam falls asleep for the first time in a week. But afterwards he won't wake up. Dean tries everything to wake him up and grows increasingly desperate. Bonus points if Sam is caught in a dream/alternate reality, or if his soul has left his body like Dean's did in 2x01, after the car crash. (Wincest or Gen is fine).

Castiel managed to take away Sam's insanity, but he didn't really know what he was doing, so there was an unexpected side effect: as the days pass, Sam's Hell memories disappear gradually. At first, the boys think it's a blessing, but soon, Sam starts forgetting the good stuff as well, whole chunks of his life, until he doesn't even recognise Dean anymore. Cue Dean desperately searching for a solution. (Wincest or Gen is fine).

When Castiel takes away Sam's insanity, the exchange goes both ways and Sam gets some of Castiel's powers. But he can't control them, or his body isn't capable of sustaining Cas' angelic essence. Dean tries to help. (Wincest or Gen).

Sam, Dean Gen

Cas may have taken the symptoms of his hell memories but the electro-shock therapy, lack of sleep and the car accident still plague Sam after he gets out of the Asylum. Sam finds himself absolutely exhausted and hurting, luckily for him he has a big brother willing to care for him.

After escaping from the hospital, Dean decides that they need a break and takes Sam back to Rufus' cabin so they can just recuperate. Cue LOTS of bigbrother!Dean making sure Sam sleeps and coaxing him to eat (Sam tells him about the thing with the sandwich).

Because we all wanted to see this. The missing scene after Castiel kills the demon and Sam says he's not real, Sam had to of moved from that bed to his room again. That's where Dean comes in.
