It isn't until weeks later that the symptoms start, and it isn't until days after that, when Sam can't breathe, that they finally understand the consequences. Hanta virus.

How do he and Dean work out proximity and forgiveness in the shadow of a no longer tenable past involvement? If you wish, let Sam go on to a tentative sexual relationship with someone else (Cas or not!dead!damn!it!Kevin or Jody would be my favorite picks), and look at how Sam and Dean's relationship weathers that development. Basically, I want to see Sam's sexuality impacted by his experience of body/self/consent violations, combined with post-wincest Sam n Dean.

Trying to not panic as he waits for Dean to get back to the Batcave, Sam realizes that he feels exhausted and the longer he's in the trap the weaker he gets, these traps were designed to weaken demons not just hold them.

Dean thinks having a fluffy little were-kitten for a brother is hilarious, and amuses himself by trying to startle Sam on the odd occasion just to watch it happen - but one day he legitimately upsets Sam enough for him to transform, and then can't find him afterwards.

In which Dean tried to get Sonny to get Sammy from John, but it failed, and John disappeared with his younger son and was impossible to trace. Dean tried everything, but John was a seasoned hunter and when he wouldn't want to be found, he woldn't be found.
After 12 years, a tall, bulky man with leather jacket and scars on his face arrives in a black Impala to Dean's nice white-fenced house, and says their father is dead. Sam is in a very bad place- John was cold and distant towards him, but still he was the only person close to him for more than decade- so Dean, seeking forgiveness, offers him shelter. Dean barely recognizes his little brother at first, but then slowly he discovers Sam is still Sam: smart, a bit obsessed about anything he does, empathic, sometimes bitchy. And that Sam forgives him, more- he was never blaming him in the first place.
Dean is happy he got his brother back, he would like Sam to stay in this town, or go to college, but as time passes, Sam becomes sadder, more distant, talks about leaving despite showing no intent to continue hunting, and starts to polish his weapons more often... Only when something drastic happens, Sam admits what really happened: that night, Azazel possessed John, sure that Sam won't be able to shoot him. He was. And his father's last words were pleads to do it and revenge Mary, but also a warning that Sam is a monster that will destroy the world one day, and he should use the next bullet on himself.

(BTW, since Azazel was killed early, gates of Hell were never opened, Apocalypse was stopped.

Sam never got his anti-possession tattoo put back on after Cas burned it off to let Crowley in to help him fight Gadreel. When they finally go up against Abaddon, Dean is completely prepared to take her down - but he hadn't planned on her vacating Josie's meat suit in favor of possessing the only person who would satisfy the Mark's desire for fratricide.

Would prefer no character death, because there's been more than enough of that over the years. Extra points for after Abaddon vacates (bec of spellwork, special MoL exorcism, whatever you have to make up), Dean realizing from Sam's terrified reaction just how deeply rooted his aversion to possession is, and why he's been too angry to do more than lash out at his "partner."

Dean was always Sam's protector, John just really wasn't up to the task for lack of practice. I'd love to see an exploration of what damage could have come to Sam had Dean not returned to his family in Bad Boys. I am especially interested in Dean finding out (from Bobby perhaps?) and coming to little brother's rescue. (if only in a comforting capacity) Also, Dean realizing what he had in the original scenario and that he belonged with his brother, regardless of the appeal of the new found life he is leaving behind.

S9. After weeks of unsuccessfully trying to subtly mend the rift between them, Dean realizes that his last chance to get Sam to forgive him will have to be a much bigger gesture. There's no point in continuing to play the blame game, because they're both at fault and both equally stubborn about holding grudges.

Sam wakes up on his birthday to a puppy (any breed, you choose) sitting on his bed, an enormous bunch of Superman balloons attached to its collar.

And where did Dean even find a card that says "Sorry I let you get possessed by an angel"??

Sam was tired and sore.

He ached in places that he hasn’t felt hurt in a long time. It was a stupid hunt that was supposed to be easy. That turned so wrong. There was right instead of left. There were missteps and mistakes at every turn. They were misreading each other and their case. Instead of a black dog, it was a ghost. They stumbled over each other like that they haven’t done in years. Even through hell, heaven, and purgatory, they always seemed to come together.

This time is different and it hurt. It wasn’t about deals or saying yes. It was matter of a choice which he didn’t have and having another angel in his body….under his skin. The moment that the angel was truly gone, he went right to shower in hopes of scrubbing the feeling away.

Even now… he had that urge.

Most of all, it made him angry. So angry…he wanted to hit and shake his brother to make him understand. He wanted to scream but he couldn’t. He couldn’t look at his brother some days.

God, he never wanted to feel like that about his brother.

He wanted to forgive but every time, he thought about it. It just made him so angry and so tired. It was an endless cycle of blame and angry. Then there was whole mark of Cain that pissed him off even more. He thought Dean would learn from him and his mistakes but it didn’t like that was going to happen.

He wanted it to stop but he had no idea how. Words just kept coming out and haven’t stopped on both sides. Both of them are stubborn and hold grudges like a dog with a bone. Now, he wanted to curl up into a ball and sleep forever.

Then he felt something pressed against his chest. He groaned. He really didn’t want to get up. Maybe, if he ignored Dean, he would go away. Then he heard an odd noise. This better not be a prank. He wasn’t in the mood especially today.

Then the noise happened again. This time…he heard clearer. He opened his eyes to see a puppy pressed against his chest. He blinked again.

The puppy was still there.

He rubbed his eyes to make sure that he wasn’t dreaming.

Nope…the puppy was still there. The puppy tilted his head and gave him this look. The puppy was cute like most are. He was beagle judging by his size and color. He laughed softly as he reached over and scratched the puppy’s head.

That is when he noticed the Superman balloons tied to his collar. He also noticed a card hanging from one of the strings.

He laughed.

“So, your name is Superman, huh?” He said as he scratched the dog harder. The dog wagged his tail happily.

He grabbed the card and pulled it down. He opened it and it read: Sorry that I let you get possessed by an angel.

“His name isn’t Superman, by the way.” Dean said from the door way. He looked up from the card and stared at his brother.

“What is his name then?” He asked.

“According to the shelter, his name is George but the last five hours that I hanged out with him. He is definitely a Houdini.”

He chuckled.

“What happened?”

“He figured out how to get into the trash with the lid on, escape from the area that I closed him off in, and figured out how to get on the table. The boy is too smart for his own good…kind of like someone else I know.”

His throat tightened up at the words. He felt this rush of emotion run through him. He didn’t know what to say to this. Dean was trying and he should too.

“So, Houdini, huh?” He said.

“Sounded a good name.” Dean said with a nod.

“It does.”

“And he is your responsibility. I am not cleaning up any poop.”

He grinned slightly as he look down at the dog on his chest.

“Okay.”

“Okay?” Dean said in confusion.

“It’s okay.”

Dean just stared at him for a moment then Dean’s face started to lighten up.

It was a start. He still needed to figure out what to do about the million other problems that they had…starting with the mark of Cain.

But with Dean smiling and a puppy here…just maybe…it will be okay.

Season 9, gen or whatever you want it to be
Abaddon has knowledge of MoL member... So when the things got very hot and Dean was about to ice her, she made a little portal, to 2005 or 2006, in an exact place she knew Dean will be, to finish it before it begun. Dean crosses the portal after her... Both Deans.

Current Dean arrives just to see his brother getting possessed by her. Somehow he manages to get rid of her, leaving Sam injured or really traumatized. (Maybe angels will arrive to stop the disruption in the Apocalypse plans?) Anyway, he has to take care of season1/2!Sam, and he notices how his little brother used to be, how he laughs, doesn't avoid touch, argues like he still believes Dean will play his music once or stop throwing socks on a floor... But he's smart as much as older Sam and doesn't buy Dean's answers about what happened to them in the future. Dean starts to think.
In the meantime, Dean, who went through the portal because he was drunk or in some shock or whatever, meets season9!Sam. For some reason this grim, beefy, hippie-haired version of his older brother won't talk to him too much. Dean is a bit annoyed by boredom (no spoilers allowed for him!) and, deep in heart, anxious. At first he was happy that in the future he still hunts with his brother, he still got an Impala, and they have a bigass awesome bat cave to boot, but then he notices something wrong had to happen to Sam, and that despite obsessive search for the spell that will bring future him back, Sam doesn't want to even look at his younger version. He doesn't know how hard Sam tries to not just hug him and cry, revealing that the future, and their relationship, indeed haven't went very well.

mid-season 9, slight AU, gen or whatever parings you'd like.

Sam feels happy. Everything is OK, he hunts with Dean, Cas is safe in the Bunker, angels and demons are in a controlled situation, there are no bumps on the road.
But he feels something weird, like static, like the voice of an angel. He starts to notice that reality sometimes is... inconsistent? He starts taking notes... but the notes also are change, when he re-reads them? Finally, he wants to test himself the way Dean taught him: by pain. With panic he notices it feels like the Cage, not like the real life pain. To make himself really really sure, he burns his hand pretty bad.
Dean and Cas soon notice weird behavior, or the marks, or his worse performance on the hunts, and they start hunting together, leaving Sam to research, (which makes him feel worthless and a tiny bit jealous), eventually they're so worried by his deteriorating state (as he obsesses to find the proof he's right), they stop hunting to keep an eye on him (which makes him feel guilty and even more worthless), but there's nothing worse that the way they come to his room, stroke his hair gently and with teary eyes they tell Sam everything will be OK, that he has to snap out of it, that he's important to them, that it's not his fault the Trials fried his brain, that they will love him whatever happens and just want him to stay with them.
Sam is sure it's not the Djinn, he remembers the unexplainable things and lost time happening before the pain stopped feeling real, and he trusts Dean more than he trusts himself... But he feels more and more like a dead weight, the reality becomes more and more messed up, and he finally decides whether he's insane or not, blowing his brains out will be the best solution.
And then he stands eye to eye with Gadreel.

Sam is turned away when he tries to donate blood during college -- there's something Not Quite Right that they just can't figure out (it's the demon blood, although Sam obviously doesn't know it yet), so he waves it off and leaves the donation center, spending the rest of the day full of anxieties and confusions having to do with his feeling inherently impure. Maybe someone (Jess, Dean, a friend, a stranger?) is able to cheer him up a little and distract him from his insecurities.

Sam has been hard of hearing or deaf since childhood (your choice of reason). When he was diagnosed (or the Winchester discovered what happened) John hung around the hospital long enough to learn basic sign language, maybe a semester at most. Dean got a little more, but he was still a relatively little kid. Sam learned to sign from his dad and brother, which means that he speaks some pigeon version of ASL, comprehension of which may or may not be improved with alcohol. He reads lips okay, but not well. Maybe he has a hearing aid, but it doesn't do much. He doesn't like to use his voice, because he hasn't had much speech therapy and people (Dean) have suggested he doesn't speak well.
Now at Stanford, Sam is in a bar. There's a cute someone, but he's too afraid to go introduce himself because he doesn't know how they'll communicate.

Bonus if the cute someone knows sign language.
Extra bonus if the cute someone is confused by at least one of Sam's signs and accuses him of being drunk.
Triple Bonus points if Sam hasn't been drinking at all.

AU where Sam gets a vision when he's a teenager of Dean dying (and it's his fault in some way), leading to him running away and possibly getting hurt himself. John and Dean eventually find/save him. He won't tell them why he ran away, but this further cements his desire to go to Stanfod.
