THE RULES
→ Leave a comment here with your prompt. This is a Sam-focused hurt/comfort community, so Sam should be the one in the hurt/comforted role. Your prompts may involve sick!Sam, hurt!Sam, angsty!Sam, basically anything that results in Sam being on the receiving end of hurt/comfort. (No rules against whumping other characters as well, of course, but you have to whump Sam first. ;) )
→ In your prompt, please state your desired characters or pairings. All genres/pairings welcome, but no real-person (RPF) prompts.

Example A: “Sam, Dean, gen, set in season 2. Sam has a vision and passes out. Cue caring!Dean and limp!Sam.”
Example B: “Sam/Dean, trauma. Sam is injured on a hunt, Dean freaks out when he thinks Sam might be dead.”

The electro-shock 'therapy' that Sam received from the demon left him with holes in his memory -- he knows who he is, and who Dean is and how he ended up in a psych ward, but some things, like the fact that just about everyone they knew or ever loved is dead, have been forgotten. Castiel says he can "shift" Sam's Hell memories, and that his solution will get Sam "back on his feet." "Back on his feet" to me implies a kindasortamaybe fix, one that isn't perfect. Maybe Sam can function better that he could before, and can generally get around, but he's still not quite alright. It's up to you how severe the 'not-alright' is. Dean has to come to terms with the truth that his brother is never going to be perfectly mentally well again, but he's awesome about it. Gen, please.

Sam/Dean. Sam's not hallucinating anymore but he's remembering things that happened to him in the cage more clearly without the constant interruption. So one morning Dean gets back from grabbing breakfast to find Sam standing naked in the bathroom carving into his body where he knows he's supposed to have scars from Lucifer. Bonus points if Sam, very calmly and very sanely, explains that really, Dean, everything's fine, he was meant to be this way.

Sam is in the hospital. A staff member / visitor / another patient notices that he has dissociative tendencies. Whenever the stress gets too much, or certain things come up, he becomes slow, disoriented, sleepy, and uncoordinated. He can't remember basic things and maybe slurs a little and is generally just... a little out of it. When it gets bad, he may even become unresponsive, drifting somewhere in his head. But he is still trying to hold it together, because... you choose: Dean is around? He vaguely knows that he needs to do something important?

Dean doesn't seem as tired at the moment, he's actually a little happy, and Sam thinks it's the best thing to come out of all this mess.

He drinks less and talks more. With this air about him of something finally went right, and Sam can't agree (we left Cas back there in that state, in my state) but he can't find it in himself to bring anything up, he's done that enough lately, he needs to stop thinking so much.

So when Sam wakes from his second sleep screaming and a heart rate faster than a rabbit's, none of the nightmare forgotten and half of it still mutely playing out around him, Dean's simply there with warm holding hands and a shushing mouth. He's calm and moving in front of Sam like a shield, filling up his entire vision, as if he already knows it's still there, still twisting and warping—

"Shhh, Sammy." Soothing. "It all alright, okay? This is good, you know, we knew, we knew, you're sleeping. That's all that matters right now, you're sleeping."

And when Sam finally breaks it to his brother that Lucifer never leaves, that he's always in the corner of his eye, just standing, never saying anything, staring into Sam like he's waiting— Dean takes that all in stride.

"Is he saying anything?" He steps in front of the hallucination unknowingly (subconsciously).

"No."

Dean grins like he hasn't in months, surprising Sam, "That's great, man. That's so much better. Just don't... don't pay him any attention, you'll be fine."

But Sam's scared Dean'll get too accustomed to fine, settle into this soft acceptance just in time for it to fall, "But what if it gets."

"I'll take what I can get," he growls, still somehow holding the grin, "and it won't. Sammy, it won't."

He knows Lucifer is just there, right behind his brother, but he allows himself to hope just a little.

Even when things get rough (Sam doesn't expect anything less), Dean drives steady.

Lucifer changes sometimes, ever so slightly, ever so grotesquely. He looks more like before (say yes), deteriorating skin and hollowed eyes. Sometimes shadows so dark fall over them Sam can't even see the whites, just two skeleton holes gaping back at him. Sometimes bloody hands, sometimes bullet holes in his shirt. Never really doing anything, just. Behind the door frame in the motel room. In the neglected corner of the laundromat. Behind the next register over at the grocery store. Always in Sam's view, no matter where he turns.

Things get rough when Hell sometimes leaks out after the sun rises.

It's when one moment, Sam's lounging in the passenger seat on a long drive through browning Kansas fields, and the next, it's just dirt and pockets of soundless fire.

It's when one trip to the department store, Sam heads to checkout and the in-store music stops playing and everyone just drops.

It's when one bright evening, Sam is mottled and torn open by a black dog amidst a row of concrete warehouses because just a second before he saw it burst into flames and rip out its own intestines. (In all honesty, he wasn't his best that day, after trying to avoid sleep and food.)

But each time, Dean drives steady. With a strong grip and comforting gaze, says, It's fine, Sammy, just your head messing up a bit. It's alright, just ignore it, Sammy, it's not real. It's okay, Sammy. Honest mistake. I'm gonna patch you up. It wasn't your fault. Cups Sam's cheek and runs a hand to get the hair out of his face and forces Sam's chin up as he's all gasps and glazed eyes and pain.

"From now on, you think something's off, you get into defense. From now, unless it's a hunt, you don't hear it, you ignore it. It ain't the best we got, but it'll work right now. It'll work."

Funny how at this moment of blood and slippery flashes, Dean's the most solid, holding Sam together with a firm palm when he's sliding out of his own flesh (gory) and mind (dark). But through the thrashing gauze Sam sees Dean, and knows he's strongest when his knuckles are pure white on the steering wheel.

They develop that system into something that works. Sam doesn't hear it, he turns away. Doesn't get caught so often in Lucifer's haunting stare. He eats, he sleeps. (And maybe he throws up and wakes covered in sweat and shouts, but, as Dean would say, he's eating and sleeping and that's so much, so much Sammy.)

And Dean stays close. Reminds them both to not think every once in a while, reminds Sam of how he's doing and how goddamnit Sam we're making this work and you're doing so much better and I wouldn't wanna be anywhere else so stop looking like you're gonna fall apart any minute because I've almost filled up my chick flick quota and the year's still young. He's happier now and Sam doesn't want to take that from him anymore.

So they're getting along. They have a lot to do, a lot to make up for. A lot of loss to crush them down. And there are some nights that are quiet and peaceful and Sam hears his name, whispered, looks up and sees Lucifer still and shadowed and quiet and smirking. And sometimes Sam's heart starts to trip up and sometimes he's lost.

But Sam knows it wasn't going to be perfect. And anyway, everything's a lot quieter now.

Dean drives steady and Sam watches his brother.
