Sam has a chronic illness--CF? Diabetes? whatever floats your boat--he's been dealing with since birth or very close to it. John and Dean did their best and Sam assumed that was the best it could be. He gets to Stanford and finally schedules his first doctor's appointment, expecting just a prescription refill, and meets a doctor who's really horrified that Sam's settled for the condition he's in. Sam has a lot of feelings about the fact that he could have been feeling a lot better for the past eighteen years.
