Sam is horrifically disfigured (as in barely recognisable, facial burns, severe scaring, maybe his skin is melted from something like an acid attack) and can’t bear to leave the bunker or be seen in public. Even around Dean (and CAS if you choose to include him or any other secondary characters) he hides himself away as much as possible.
