41- Non-Con. Season 10. Gen.
Dean has to help his brother come back from something much darker than a shoulder injury. When Cole tortures Sam he also rapes him as a way to get back at Dean since Dean isn't giving into his demands. Dean later learns what happened when he was beating up Cole for a second time and the man lets it slip. Sam has to stop Dean from giving into the mark and killing Cole and Dean has to somehow pick up the pieces that's been shattered of his brother.
42- Gen, season 6. After a huge fight, an angry Dean takes off. Normally, this wouldn't be a problem, but Sam suffers a huge seizure right after and Castiel has to take care of him. Sam is convinced he's too broken and that Dean would be better off without him. Castiel shows him otherwise. Claimed by

Cole tracks Sam and Dean back to their bunker and intends to do some ret-con to get back at Dean. What he finds however is Dean taking care of Sam due to the shoulder injury he had when Cole originally kidnapped him. Cole has issues seeing Dean being so gentle and caring with Sam, compared to the Dean he remembers as being a cold-blooded killer. Even when he's able to get close to Sam to take him out, in order to inflict the same feeling that Dean did to him, he can't bring himself to do it.
46- Sam knows addiction. He knows power. He knows betrayal. Sam knows regret. He knows frailty. He knows faith. The best and worst choices he's ever made were to save/protect/help/encourage Dean. He just hopes that this one will be the last. Claimed by

47- How did Sam end up with a major like tax law? Especially when he's so good at Latin and ancient texts, digging around in corpses, and assessing suspects.
I suspect Sam went through a number of majors before he settled on one that required smarts and a fantastic memory, but didn't remind him of his family or hunting at all. It was just too painful.
48- Season 10.
If he thought it would change anything, Sam would have told Cole not to bother. Vengeance is empty. Dean is dead. And Sam's been tortured by the best. Pain is relative. Cole doesn't know how good he's had it, but if he doesn't stop now, he'll learn.
49- S09 AU: Every time Dean gets it in his head to tell Sam the truth or, heaven forbid, force Gadreel out of his brother himself, Gadreel conveniently zigs when he should have zagged, takes a nap, or just plain can't be bothered to to keep Sam's lungs expanding. Like his own personal "Mystery Spot," watching Sam suffer over and over again keeps Dean in line...until it doesn't. The boys work to rid Sam's body of it's angelic life support, regardless of what the outcome means for Sam's health. Claimed by

50- Sam takes a bad fall. Dean wants to blame the poltergeist in the faded old hotel, but really, it was Sam's Sasquatch foot on a mushy spot of rotted flooring that sent him plummeting down several floors to the marbled lobby. He looks like he's making a snow angel in the dust, and there's a halo of blood and who-the-hell-knows that makes Dean's stomach clench, but he's alive. Blind, broken, bleeding...but alive. 51- In the season 5 finale we saw Lucifer show Sam just how many people in his life had been possessed in order to ensure he stayed on the right track - from teachers to prom dates and many others besides - all chosen to be possessed so they could manipulate Sam's early life.
Pick one person from Sam's pre-series years and tell the story of how they influenced Sam through causing him some kind of pain or loss. Pain can be emotional or physical trauma, but designed to keep him on track to become Azazel's champion. The demon(s) - can be one demon who did all the possessing or many different demons - were hand-picked by Azazel to complete the task. One reason why so many people were possessed along the path of Sam's life was because Dean was usually there to save him, and the other because Sam is so damned stubborn and determined to run his own path that he needed regular, sometimes brutal, course correction. The story can be told from Azazel or the possessing demon's POV, or not.

52- While at Stanford, Sam contracts mono and fails the semester, losing his scholarship. With no means to stay in school and no family to fall back on, he makes an attempt on his own life. Bobby/Pastor Jim/whoever is called, and must track down Dean and John. Their reactions are up to you, but please end with awesome!Dean getting Sam back on his feet.

53- The boys catch a case where men and women of various ages and walks of life are being boiled alive from the inside. They soon connect the dots back to an angel who is set on destroying Azazel's blood-line by casting a spell to boil off impure blood. Dean's ready to cut the rogue crusader off, but Sam's not convinced it's a bad thing...and then, of course, he falls under the spell. Prefer Gen but will take Wincest. Some fun/awesome!Cas is always welcome too!
54- Fic or art welcome! Any season, but S3, S7, or S10 preferred. Gen or slash. Sam and Dean are hunting a night mare. Sam falls under its spell, and becomes locked in an unending series of his worst memories and fears made vividly real by his subconscious. Dean takes dream root to go into Sam's mind to try and get him out, but the night mare is having the feast of its life, and won't easily let Sam go. Claimed by

55- Spoilers season 10
Mark of Cain Dean does end up killing Sam, thus curing him. The only way Dean agreed was that Sam's "death" was only supposed to be temporary, however Able (or someone) is keeping Sam in Heaven. In order for Dean to get Sam back he and Cas need to go to heaven and get him. Lots of scenes of Sam's REAL heaven of brother bonding and the friends they have made over the years. Happy ending please.

57- S4: Sam avoids using his powers. An exorcism gone awry leaves him bleeding out. All things considered, it's not the worst thing ever-- Dean thinks. At the hospital, the doctors are confident that Sam will be fine after a transfusion, but as the night drags on, Sam doesn't bounce back. In fact, he seems to be getting worse. He's not rejecting the donor blood. They've got tests for that, and it's not it. The doctors don't know why Sam went from a little low in the tank to seizing and hallucinating in just a few short hours. Castiel won't answer Dean's summons. Bobby doesn't like it, but Dean's THIS close to calling Ruby. Claimed by

Dean thinks it's a little weird that Sam seems to be just fine, but his let's-talk-about-it-emo-little-brother appears to be functioning all points normal, recovering physically and with no real emotional repercussions of Dean's stint as a demon.
Until one night a few weeks later, when somebody Dean ticked off as a demon catches up with them. Dean's more annoyed than anything else, because the idiot was stupidly easy to disarm, and shallow stab wounds hurt like hell, and he's never going to get the blood out of this shirt.
But Sam? Sam freaks.Out. Dean gets a first-hand look at just how traumatizing his death was - and how even being a demon doesn't erase the knowledge of how to comfort six-odd feet of panicking little brother.

59- The angels fail to change present Dean's mind about saying yes to Micheal, even with his trips to the future and to the past. So they go back to a young Dean, just abandoned by Sam for college and they show him the future Sam and all his sins against Dean and humanity. As past!Dean learns more and more, it begins to effect present!Dean, much to confusion of a very hurt Sam as his brother seems to turn on him without warning.

60- Zachariah puts the boys back together after Cas comes back but he doesn't have to do a good job of it and ever since Sam has developed deep aches and pains in his legs where they were broken and as time goes on, it slowly becomes near debilitating.

61- Season 8. Dean is pissed. Not only did Sam leave him to rot in Purgatory, but also one month after his return, Sam disappeared for a whole week, leaving only a note that says he had something to take care of. Now one month after that incident Sam disappears again, leaving the same stupid note, but this time Dean tracks him down to an abandoned house, and what he finds there shock him to the core. Sam is on the ground writhing in pain, with Benny biting his neck and drinking his blood. Benny didn't rescue Dean willingly. It was because Sam made a deal with him, that he would let Benny drink from blood for a whole year if he saved Dean from Purgatory. The week Sam disappeared was because he was hospitalized for the severe blood loss. So yeah, Dean is pissed.

62- Sam is on trial for his life, the charge... letting his big brother down. Our Judge/bad guy begins to list all of Sam’s real & perceived sins. From leaving Dean behind to go to Stanford, drinking Demon Blood, being Soulless, not trying to get Dean out of Purgatory. (Author’s choice of sins)
Dean is forced to listen to all the things that Sam still feels guilty about. Sam offers no apology or defense for his ‘crimes’.
Dean comes over all protective and begins to defend Sam, realising through the process that there is an awful lot that Sam has kept hidden from him, like the real reason behind so many of his ‘sins’

63- AU. Sam/Castiel. Sam is in a mental hospital and Castiel is the psychiatrist that sees how good and sweet he can be when he is in his right mind. Sam doesn't understand why he is drawn to Castiel but sticks close to him, hoping he'll get better and that he and Castielcan make it work.
64- Gen. Weechesters. Sam has chicken pox and Dean, being a great big brother, cares for him: bathing him, feeding him, making him feel better.
65- Remember the Alpha Vamp? Remember how he thought Soulless!Sam would make the perfect beast, and then never made good on that threat?
What if he came back around and found a recently re-souled Sam, with his freshly walled brain and weak from being on nothing but IV fluids for however long it was? What if he turned Sam and the wall came crashing down? WHAT IF?
66- demon!Dean, bloodjunkie!Sam: Demon!Dean doesn't want Sam dead, not really. So okay, he goes after one-armed Sam with the hammer and maybe ends up catching him and getting a little too, well, ambitious ... Dean has no choice but to feed Sam his blood before Sam dies. 67- There’s a rumbling through Purgatory, something in the air. Castiel feels it; Madison, Jack, Benny feel it; but most of all, Dean Winchester feels it. And it feels like home; it feels like Sam . (AKA Sam does look for Dean in Season 8 and finds a way into Purgatory.)
Sam has been running himself ragged trying to get into the place, though, and he's in no shape to be fighting monsters by the time he crashes, head-first, into this ominous new world. He finds himself stalked by all manner of monsters who recognize him, and hold monster-sized grudges. Will Dean get to him in time? And when he does, how do they get home again? Claimed by

68- Season 10.
After Dean's comment about the sling making him a crybaby, Sam decides to stop wearing it before his shoulder is fully healed. Some small, everyday task (carrying his duffle, falling out of bed, etc) reinjures it. Cue a guilty Dean making sure Sam doesn't have to do anything to possibly hurt it any further (and showing him that he still wants to take care of him after his stint as a demon).

Sam's smart. He knows the stuff Dean said to him while tied up in 10X3 was demon junk. He knows Dean doesn't think any of that. So when a demon on a case repeats the same lines, word for word along with "he meant every word", why does it bother him so much? In short, Sam being bothered because there's that part of him that thinks "maybe he meant some of it," even though that part is small.
72- Season 10 spoilers.
Sometime after the last episode, Sam is taken by a monster. Any monster. He's there for about a week and it sucks it really does and he's starving and beat up, but he's had worse. He just has to wait for the cavalry.
In short; Sam is kidnapped and the pressure of saving him/ getting him out causes Dean to lose control of the mark a little. Cue an injured Sam trying to talk him down.

73- John is distant after Mary's death. He's zoned out, distracted and forgetful. So much so that the date of Sam's doctor's appointment slips by him in the aftermath of the fire. Sam misses a couple of pretty vital vaccinations - including the pertussis vaccine (whooping cough).
Everything's fine for a few years. Then, suddenly, a five- or six-year old Sam starts showing symptoms. Since its first signs resemble the common cold, John shrugs it off. How far it progresses (a good list of the symptoms and progression of the disease can be found here) is up to you.
74- Malnourished/thin/starved!Sam. Any season or preseries. Maybe John didn't leave the boys enough money, or the Stanford scholarship doesn't cover summers, or some baddie decides taking Sam captive is a good idea. Cue Dean gradually getting his weight back up and just generally taking care of him.

75- Sam still has some of Gadreel’s grace left in him. The grace starts to decay, because Gadreel is dead, and Sam becomes very sick. Dean takes care of him, and tries to find a cure.
76- Season 10 spoilers.
Sam has dealt really well so far with Dean as a demon and the First Blade and Dean killing Cain, but one night something just snaps and Dean finds him the next day after several hours of avoidance because he thinks Sam is afraid of him now in a near catatonic state curled in a fetal ball on his bed. Dean digs deep and finds that he still has it in himself to be the same loving big brother who raised Sammy and gently talks him back to life.
77- Season 9-10 spoilers.
Sam would rather die than be possessed again. Or so he thought. But what if Cas's fading grace causes him to need to temporarily take a vessel with a soul (assuming Jimmy's soul has left the building) to recharge in order to survive...and the Mark of Cain makes Dean an unsuitable host? And it's an emergency, no time to find another willing human.
Cas is Sam's friend and he won't let Cas down, regardless of his own personal trauma. How does Sam deal with sharing space with Cas? Perhaps Cas shares some of his gained insight on Sam with Dean, to help him understand his brother better...

78- Set in season nine before Sam knows about Gadreel.
Dean gets the stomach flu and knowing how Sam's immune system is weakened from the Trials and still healing, he tries to keep his distance so Sam doesn't catch what he's got. He stays in his room and pushes Sam away till a few days later when he's feeling better. Unfortunately Sam picks it up anyway. The bug lasts so much longer for Sam and is twice as bad because his body is weakened. Maybe Dean confronts "Zeke" and tries to get him to heal Sam faster. Either way Dean is worried and Sam feels miserable. (Would love the bug to have lots of vomiting and diarrhea and embarrassed Sam bc he's got it coming out of both ends at once and needs deans help to stay upright)
