This gives everyone time to write and prompt to their heart's content, and allows us mods to keep up with y'all. And we're starting right now!

Go wild: anything from nausea and dizziness to psychosis. Maybe strong rhythms, like waves or music with a regular beat, help ground him. Or sex, if you wanted to go the pairing route.

Post-6.22. One comparatively minor issue in the mile-long list of all the ways Sam's been screwed up by the destruction of the Wall is the fact that he can't shave himself anymore. But it's eating at him, not just because he's irritated by the way being unshaven looks and feels so much as it is the principle of it-- the fact that he's so freaking scared to do something that used to be so easy before. But so much as touching a razor freaks him out because of all the unpleasant associations he now has with razors and blades in general, let alone scraping one across his face, and he can't get over it. Dean realizes it's upsetting him and decides to help out.

Cue grudging compassion and Adam dragging Sam around the country, hitchhiking with truckers and stuff while trying to keep Sam's lunacy on the down-low, in search of anyone else who wants to take him, anyone at all. When he finally gets his chance to be rid of Sam for good and for keeps, he barely has one foot out the door before he notices Dean/Bobby/the psychiatric nurse/whoever trying to calm Sam down and doing it all wrong. Breaking up is so hard to do.

Post-wall: It’s not that Sam is hallucinating so much as he’s now capable of seeing into other dimensions. Which, of course, to the untrained eye, comes off as Sam hallucinating. Dean, unfortunately, is an untrained eye, and the inter-dimensional being who might have formerly conveniently let Dean in on the secret is currently off playing God somewhere. So, Dean is going to go with the “Sam hallucinates ALL THE TIME” theory and acts accordingly. Does he ever figure it out?

What torments Sam most about his memories of the Cage, after he gets a tenuous grip on the flashbacks and fear, is how he broke there: in the worst way a Winchester could possibly break. He remembers wishing Dean was there instead of him. Dean assumes that Sam is miserable and ashamed because of something done to him, instead of anything he'd done, and Sam is inconsolable. Does Dean ever figure it out? Can Sam ever move on?

As many cellars, abandoned houses, rotting warehouses, and other manky spaces as these guys have run through, I figure their chances of getting tagged have to be pretty high. Sam has a run-in with Lactodectrus mactrans, which in turn leads to all the fun and joy of latrodectism. Dean's there to do the motel rool triage (and the big brother thing) when the agonizing full-body cramps set in, of course. Mega-super bonus points and my eternal love for some kind of background plot, especially if it gives Sam occasion to be awesome, but mainly, I'd just love to see him in agony from Black Widow venom.

After being in the Cage, Sam can't even really tell when he's been hurt, no matter how bad it is. He'll get hurt in a hunt and not know till Dean freaks and tells him he's bleeding. He'll wander off barefoot, lost in his head, and Dean will wake up and run after him. His feet will be all cut up and bloody and Dean's panicking that he doesn't know what's wrong with his brother and doesn't know how to help him. Gen preferred.

Thus, Dean has to gently remove his clothes, get him into the shower, get in after him, and bathe him. Sam, still very much out of it, is highly compliant. Afterwards, Dean gets him dressed and into bed. Maybe he cuddles with him, and/or tries to coax him out of his stupor.

Another Point of View
Post-wall: It’s not that Sam is hallucinating so much as he’s now capable of seeing into other dimensions. Which, of course, to the untrained eye, comes off as Sam hallucinating. Dean, unfortunately, is an untrained eye, and the inter-dimensional being who might have formerly conveniently let Dean in on the secret is currently off playing God somewhere. So, Dean is going to go with the “Sam hallucinates ALL THE TIME” theory and acts accordingly. Does he ever figure it out?
Metronome
OK, this is an odd idea I've had kicking around since 5.22. What if the cage were timeless, or outside of time, and returning to time after being timeless had major repercussions? Physiological, where all Sam's heart rate and breathing rhythms and such were fouled up; psychological, where he no longer knows how to follow a conversation or text or train of thought consecutively without jumping about.
Your Name in Lights by
What torments Sam most about his memories of the Cage, after he gets a tenuous grip on the flashbacks and fear, is how he broke there: in the worst way a Winchester could possibly break. He remembers wishing Dean was there instead of him. Dean assumes that Sam is miserable and ashamed because of something done to him, instead of anything he'd done, and Sam is inconsolable. Does Dean ever figure it out? Can Sam ever move on?
Along Came a Spider by
As many cellars, abandoned houses, rotting warehouses, and other manky spaces as these guys have run through, I figure their chances of getting tagged have to be pretty high. Sam has a run-in with Lactodectrus mactrans, which in turn leads to all the fun and joy of latrodectism. Dean's there to do the motel rool triage (and the big brother thing) when the agonizing full-body cramps set in, of course. Mega-super bonus points and my eternal love for some kind of background plot, especially if it gives Sam occasion to be awesome, but mainly, I'd just love to see him in agony from Black Widow venom.
Lest Thou be Consumed by
After being in the Cage, Sam can't even really tell when he's been hurt, no matter how bad it is. He'll get hurt in a hunt and not know till Dean freaks and tells him he's bleeding. He'll wander off barefoot, lost in his head, and Dean will wake up and run after him. His feet will be all cut up and bloody and Dean's panicking that he doesn't know what's wrong with his brother and doesn't know how to help him. Gen preferred.
Unable to Stay by
Sam is a bit shell-shocked after a hunt (maybe an innocent person dies? Someone he knew/got to know?) and has blood on his hands, arms, and face. Thus, Dean has to gently remove his clothes, get him into the shower, get in after him, and bathe him. Sam, still very much out of it, is highly compliant. Afterwards, Dean gets him dressed and into bed. Maybe he cuddles with him, and/or tries to coax him out of his stupor.
Just Following Orders by
Sam shoots his father. On purpose or accident -- repercussions are going to be severe in any case.
You Never Forget Your First by
The boys are arguing. And this happens to be the one and only time that Dean ever takes his anger out on Sam in a violent way. Maybe it was an accident. Maybe he was really drunk. Maybe he was cursed. Maybe he was just really angry and he screwed up. The thing is, Sam actually gets hurt, and it's Dean's fault. Cue massive guilt and general awesomeness afterwards.
Brownie points if Sam thinks he deserved it for whatever reason. Or is angry about it. Or somehow scared of Dean.
The Cancer Inside by
Dean tangles with a djinn who grants you what you want most, with a wicked sense of what it probably thinks is humor. Dean's pissed at Sam, angry, just wants Sam to shut up and listen and grovel maybe a little bit more because hey, the guy jumpstarted the apocalypse and betrayed Dean. So yeah, Dean wants that. And in this world, Dean gets just that.
Cooler Than Me by
Wee!Sam (5-12) catches the eye of an infertile witch. She kidnaps him and casts a spell on him to make him believe that she is his mother.
Not Exactly Clarence by
Sam, Cas, gen, sometime in Season 5. Sam gets sick, and Cas is the only one who notices. Sam is ignoring the problem until it goes away; cue Cas trying (awkwardly) to take care of him.
To Be Redeemed by
Sam falls apart. Dean tries to help. That is all.
Draining by
Sam has the mother of all chest colds, a raging headache, draining fever, wracking chills. By all rights he should be in bed, but people are dying and bones need to be burned and Dean can't do it alone. So here they are, in the middle of a forest in the pouring rain, trying to light a stupid match because of course the lighter fluids all gone...you get the idea, right? One of those no good, pain in the ass, nothing going right, suck-the-life-from-your-bones days. Grumbling but protective Dean would be awesome.
Mother Instinct by
Season 2: The boys are forced to crash at the Roadhouse for a time. Sam is hurt/sick/whatever and in bad shape, and Ellen decides the poor boy needs some serious mothering.

He's more grounded when he can scratch an interviewee's mutt behind the ear. He even comes out of his shell a bit; Sam can be so far in his head that he can barely take care of himself and still come out of his memories enough to take care of a stray.
What the hell. Not like a dog is close to the worst thing they've ever smuggled into a motel room.

During the year he spent alone a hunt with Sampa went bad. Sampa had a plan to use him as a bait and kill their current baddie while it's distracted. Except the plan went awry and Not!Sam got stuck with the MOTW for days or weeks (up to the author) while it tortured him (again up to the author about how and how badly he is hurt etc...)
When Sam is brought back and he gets all of Not!Sams memories he is the one who has to deal with the trauma as Not!Sam never did. Maybe the baddie was never caught and it's gunning for Sam again. Cue hurt/vulnerable Sam and protective/awesome Dean.
All right, shutting up now as I am still working on my own fill for the last meme...

Now, with Sam back from Hell, re-souled, and his wall detonated, something happens to remind Dean of Sam's old guitar-strumming days (maybe they work a case in a music shop or something). Sam has trouble staying in the here and now but Dean figures out the guitar helps ground him. Maybe he tries to commit him to it via booking him a small gig at a coffee shop, or something, and it goes well, or it goes really, really badly. Or maybe he goes for something more intimate, and gets Sam to teach him. Wherever you wanna go with it. Any pairing/genre.

Dean, not knowing Sam's had it all this time, has no idea why Sam is acting like a freak of nature and worries accordingly. I'd like a twist on this one, though. Something like Dean not finding out at all, and Sam continuing to he a secretive weirdo. Or Dean finds out and Sam doesn't want to give it back to him because he doesn't trust him not to throw it away again next time he's feeling crabby.

He fumbles the glass and it crashes into the sink. It doesn’t shatter but a crack splits from the heavy base clear up the side, and a tiny chip falls away somewhere. Sam doesn’t know where. Light flashes along the crack as he sets the glass on the sink counter with a lightly trembling hand.
No matter how hard he looks, Sam can’t find the missing piece.

Dean says, “Lay the hell off the coffee, would ya?”
Sam smoothes his hands over his thighs, denim biting his palms. “Sorry,” he says, “Sorry.”
Dean stares, and Sam thinks he might recognize that expression. Vague disgust, maybe. It’s incongruous. It sits wrong, not just on his face but in the room. Doesn’t match the wallpaper. Doesn’t match.
“I—Dean?” He almost bites his lip, remembers not to at the last moment. “I’m so—I’m going out. Okay? I’ll be back. I just need—”
“Sam!” Dean cuts the air with the edge of his hand. “Just…go, okay? Before you vibrate right outta your clothes, Jesus.”
He can’t keep the smile off his face (grateful, relieved), and Dean stares.
Sam scuttles out the door and grabs both his hands, holds them close to his belly.

Sam looks at the two pieces of glass in his hand. He broke it, in the end, because he couldn’t stand looking at it. At the pieces that didn’t come apart when they should have. Held it in both hands and pulled, until they gave up. Snapped apart. Easy. Like they were meant to be, all along.
He says, “There’s nothing to tell. Dean. It’s fine.”
He says, “It’s fine.
“Dean.”

“It wasn’t your fault,” Dean tells him, standing in the five a.m. sunlight, hands open and calm at his side. “Whatever it—Sam you have to know. It couldn’t be. It wasn’t.”
Sam lifts his head from his hands but doesn’t stand up from the asphalt. They’re the only people in the lot and it’s Sunday morning.
“S—” he starts, and Sam leaps to his feet.
“Shut up! Just—don’t. Don’t say it! I’m sorry. I’m sorry. Don’t say it. I’m sorry.”
Dean’s mouth hangs slightly open, gobsmacked. Jaw quivering. He licks his lips, lightning fast, and looks away.
“Sorry,” Sam says again, and turns away, wraps his arms around himself. Shivers when he hears Dean’s voice, low and flecked with dismay. Sam tightens his grip, but it doesn’t matter.
“Sam?” Dean’s voice pitches up at the end of his name, and Sam can’t help but flinch. “What did you do?”

“It was Dean,” he whispers. “Dean. It was D-Dean.”
“Please.”
Please. Oh please.

“Sorry,” he whispers. “I. I.”
“You have to tell me.” Dean’s voice is a long way off, but it carries. It was made to carry. “You have to tell me the truth.”
He stares at the pavement with dry eyes.
The light burns all the way down.

Balthazar looks at Castiel and sighs. “Hmph. A sound expressing thought, or consideration.”
“You are considering this vehicle?”
Balthazar completes his circuit of the van, pausing to admire the barbarian queen riding the polar bear. “Oh, I’m more than considering it. This is definitely… how would Dean put it? Our ride.”
“We will not be inconspicuous in this,” and Castiel’s missing the point so badly that he’s practically in a semi-circle.
“Inconspicuous is not what we’re after here, brother. We’re on a journey of discovery. “
“I’ve already discovered far more than I’m comfortable with lately. Did you have some specific quest in mind?”
Balthazar eyes Castiel up and down and stops short of rolling his eyes. He grasps the sleeve of the hideous tan trench coat and in the blink of an eye they’re halfway across the country on the grounds of a spacious estate. They rematerialize in the middle of a flock of birds, who continue to peck at the ground, oblivious to the sudden appearance of angels in their midst.
Castiel gazes imperturbably at the birds before raising a questioning eyebrow. “We are on a quest for avian life forms?”
“Not just any avian life forms, my sartorially challenged friend. We are on a quest for the regal peacock.” Balthazar sweeps his arm in an encompassing gesture. “And we have found him.”
“It would appear that our quest is complete. I had anticipated it taking a bit longer.”
“No, no, no.” A despairing headshake is the best Balthazar can manage. “Our quest is not complete. Not until we take the peacock into ourselves.”
“I do not eat poultry.”
“Not literally into ourselves …” Balthazar calms himself with effort, “figuratively into ourselves.” He waves his arms and the flock startles, tail feather popping open as far as the eye can see. They’re gorgeous, flashy and unexpectedly breathtaking. “We’re…well, you more so than I, a little drab in comparison. I can’t see two such powerful beings as ourselves being outdone by a strutting peafowl.”
Another blink has them back at the van and Castiel’s brow furrows in thought. A moment later, his trenchcoat is bright blue with iridescent circles of green, orange and gold decorating it.
“It’s a good start,” Balthazar encourages, “we’ll work on internalizing it as we go.”
